Instructions for completing this form:

1. Please submit (1) a copy of your SPM results slip and (2) a copy of your identity card.

2. An administration fee of RM25.00 per certificate is applicable and payment is to be made to LCCI INTERNATIONAL QUALIFICATIONS (MALAYSIA) SDN BHD. Please note that only bank draft / postal order / money order is accepted. Payment by cash or personal cheque is NOT accepted.

NOTE: - Eligible candidates can apply for their Certificate through their respective schools from 1st – 30th April 2011. Certificates can be collected from the respective schools from September 2011.

- From 1st May – 31st October 2011, candidates can apply for their Certificate directly to LCCI International Qualifications Malaysia o�ce or LCCI Authorised Training Centres that are listed in our flyer / website. Certificates can be collected from the respective schools from January 2012.

SPM Double Certification Project 2010

Kenali LCCILondon Chamber of Commerce and Industry (LCCI) International Qualifications dari United Kingdom menawarkan kelayakan secara global selama lebih 100 tahun. Setiap tahun, lebih 250,000 calon LCCI dari 120 buah negara menggunakan kelulusan LCCI untuk mencari pekerjaan, meningkatkan kemahiran, memasuki universiti atau menyertai badan-badan profesional.

KelayakanMulai tahun 2008, LCCI dengan kerjasama Kementerian Pelajaran Malaysia bersetuju menganugerahkan Sijil LCCI Level 2 Certificate in Book-keeping and Accounts, kepada calon yang memohon serta memenuhi syarat kelayakan dalam mata pelajaran SPM Prinsip Perakaunan dan Bahasa Inggeris 1119. Sijil LCCI Level 2 Certificate in Book-keeping and Accounts (Bahasa Melayu) akan dianugerahkan kepada calon yang tidak memenuhi syarat Bahasa Inggeris 1119. Kelayakan calon untuk Sijil LCCI akan tertera dengan jelas dalam keratan keputusan peperiksaan SPM.

Prosedur PermohonanPara Pelajar boleh memohon sijil LCCI melalui cara-cara berikut:

• 1 April – 30 April 2011 Calon boleh memohon melalui sekolah masing-masing

dengan menggunakan Borang Permohonan LCCI. Pihak sekolah hendaklah menghantar semua borang permohonan kepada LCCI pada atau sebelum 30 April 2011. Calon boleh mendapatkan sijil LCCI dari sekolah masing-masing mulai September 2011.

• 1 Mei – 31 Oktober 2011 Calon boleh memohon secara terus atau melalui

perkhidmatan pos dengan Pejabat LCCI International Qualifications Malaysia / Pusat Latihan yang dilantik oleh LCCI seperti yang tertera di halaman belakang risalah ini. Calon boleh mendapatkan sijil LCCI dari sekolah masing-masing mulai Januari 2012.

Bersama-sama ini dilampirkan Borang Permohonan LCCI untuk keterangan lanjut.
