Started in the year of 2015 in HSAJB, every Tuesday from 9.00am-12.30pm, MTAC Heart Failure is another

clinical pharmacy service under the directive and supervision of Pharmaceutical Services Division which

sees the expansion of clinical role of pharmacists in the out-patient setting. The main objectives are to

assist doctors in the patient management for those prescribed with heart failure medications, provide

continuity and enhance patient care through education, monitoring, and close follow-up besides

maximizing the benefits of therapy and minimize the adverse effects.

The clinic is an approach of teamwork practice consisting of doctors, pharmacists, and nurses which by

the follow up of patients are through doctor’s referral. Among the activities in the clinic include drug/

herbal/supplement history taking, counseling, dosage adjustment based on signs and symptoms of fluid

accumulation.

This service will be continuously assessed to ensure that patients are receiving optimal care. Outcomes of

the service will include the percentage of patients’ readmission, response to therapy, adverse events,

quality of life, and patients’ understanding towards the therapy.

Penguatkuasaan Farmasi berperanan untuk

memastikan semua produk farmaseutikal,

tradisional dan kosmetik dalam pasaran

adalah sah dari segi pendaftaran dan

notifikasi. Ia juga berfungsi untuk

memastikan bahawa pengendalian

rangkaian bekalan dan pemasaran, iklan-

iklan produk tersebut (termasuk juga

p e r k h i d m a t a n p e r u b a t a n ) d a n

penggunaannya dikendalikan dengan

mematuhi peruntukan undang-undang yang

terlibat. Usaha ini dilakukan melalui aktiviti

penguatkuasaan dan pencegahan, kawalan

melalui pelesenan, aktiviti pemantauan dan

juga aktiviti peningkatan kesedaran

pengguna untuk menurunkan permintaan

terhadap produk yang tidak selamat atau

membahayakan.

oleh Bahagian Perkhidmatan Farmasi,

Kementerian Kesihatan Malaysia.

Perlesenan & Pemeriksaan

Bahagian Pelesenan mengeluarkan 5 jenis

lesen dan permit racun kepada mereka yang

berkelayakan untuk mengimport, membeli

dan mengendalikan racun-racun berjadual

secara borong, runcit atau borong & runcit.

Bahagian Pemeriksaan & Komplians

menjalankan aktiviti pemeriksaan ke atas

premis berlesen, premis berdaftar dan

premis tidak berlesen secara berterusan

sepanjang tahun untuk memastikan pelesen

mematuhi keperluan undang-undang.

Mesyuarat BPF Bersama-sama

Risikan adalah keupayaan memperolehi,

menganalisa dan menyalurkan maklumat untuk

menghasilkan petunjuk kepada serbuan.

Sekiranya risikan berasas, aktiviti serbuan

dilaksanakan ke atas premis yang menyimpan dan

menjual ubat mengandungi racun berjadual serta

ubat & kosmetik yang tidak berdaftar dengan

Kementerian Kesihatan Malaysia.

Pendakwaan

Siasatan kes dilakukan dengan lengkap ke atas

individu atau syarikat yang melanggar

mana-mana akta yang dikuatkuasakan oleh

cawangan ini. Seterusnya pendakwaan

dilaksanakan di Mahkamah dan hukuman

dijatuhkan bagi pesalah oleh Hakim/Majistret.

Kursus Tatacara Prosedur Mahkamah telah dijalankan pada 2-5 Mac 2015 di Bilik Seminar, Hospital Permai dan di Mahkamah Majistret Bandaraya Johor Bahru bersama-sama Tuan Timbalan Pendakwa Raya bagi memantapkan ilmu pendakwaan.

dan bahan-bahan kimia terkawal yang

pengimportan yang sah sahaja dibenarkan

diimport.

Pencegahan & Kesedaran Pengguna

Unit Pencegahan dan Kesedaran Pengguna bertanggungjawab dalam

memberi kesedaran kepada pengguna terhadap pentingnya

penggunaan ubat-ubatan yang berdaftar dan kosmetik bernotifikasi.

Antara aktiviti yang dianjurkan termasuk ceramah, pameran, dialog

dan penerangan melalui radio untuk meningkatkan kesedaran dan

pengetahuan orang awam tentang ubat– ubatan. Unit ini juga

bertanggungjawab mengendalikan aduan berkaitan produk dan

aktiviti penguatkuasaan farmasi.

Siasatan

Siasatan ialah satu proses untuk mendapatkan maklumat mengenai

apa-apa bentuk bahan bukti yang di temui samada secara lisan/

bertulis. Matlamat utama siasatan kes adalah untuk mengumpul

keterangan yang relevan dan kebolehterimaan (admissible) bagi

membawa orang kena saman (OKS) ke muka pengadilan.

Operasi Bersepadu ke atas premis-p r e m i s y a n g menjual cecair e-Cigarette di Johor pada Disember 2015 yang diketuai oleh Jabatan Siasatan Jenayah Narkotik IPK Johor

Pameran yang diadakan bertujuan meningkatkan pemahaman orang awam berkaitan produk berdaftar dan kosmetik bernotifikasi. Sebanyak 77 pameran telah diadakan sepanjang tahun 2015.

Prompt antimicrobial therapy for an infection is crucial; however inappropriate use of antimicrobials has driven the

relentless expansion of resistant microbes leading to a loss of efficacy of these “miracle drugs”. Point prevalence survey (PPS),

is a practical method for surveillance and investigation of antimicrobial prescribing pattern, especially in the absence of

electronic prescribing. PPS can identify areas that need further audit in a single hospital as well as identifying differences

between hospitals in term of prescribing patterns.

A multicenter point prevalence survey which includes Hospital Sultanah Aminah and Hospital Sultan Ismail in

Johor Bahru, Hospital Pakar Sultanah Fatimah in Muar, Hospital Sultanah Nora Ismail in Batu Pahat, Hospital Segamat and

Hospital Enche’ Besar Hajjah Khalsom in Kluang was conducted in 3rd week of December 2014. During the data collection

period, patients in the ward who are treated with antibiotics will be identified by reviewing medication charts and bed head

tickets. Pertinent information will be recorded using the designed data collection forms.

Introduction

Main objectives of this study is to evaluate the

antibiotic prescribing pattern for warded patients in

the studied hospitals.

Specific objectives of study are:

To measure prevalence of patients prescribed

with antibiotic(s) at each hospital during the

study period,

To retrieve number and type of antibiotic(s)

prescribed for all patients,

To identify pattern of prescribing by different

departments,

To evaluate indication of the prescribed

antibiotic(s),

To obtain number and percentage of culture(s)

sent prior to starting antibiotic(s) empirically.

Objectives

Findings of PPS 2683 patients were admitted into 6 studied hospitals and 1045 patients were treated with antibiotics during the study

period. As shown in Figure 1, Hospital Segamat had the highest percentage (43%) of patients treated with antibiotic(s) during

the point of study, followed by Hospital Pakar Sultanah Fatimah, Muar (41.5%) and Hospital Sultanah Nora Ismail, Batu Pahat

(40.2%).

The findings demonstrated that the five most commonly prescribed antibiotics among the six hospitals were

Amoxycillin/Clavulanic Acid (21%), Cefuroxime (9.1%), Ampicillin/Sulbactam (7.5%), Cloxacillin (7.2%) and Benzylpenicillin

(6.2%) (Figure 2). This is similar to the results of the study carried out by M.A. Aldeyab et al. whereby the most frequently used

antibiotics were the combinations of penicillins and beta-lactamase inhibitors.

The antibiotic prescribing patterns varied in different hospitals. In general, Amoxycillin/Clavulanic Acid was the most

frequently prescribed antibiotics among Critical Care, Medical, Respiratory, General Surgical, Dental, Obstetrical and

Gynecological, Ear, Nose and Throat as well as Urological Departments in the studied hospitals. On the other hand, Cefuroxime

was commonly prescribed by Orthopaethics and Cardiothoracic Departments, whereas Cloxacillin was most frequently used by

Nephrology and Dental departments. Paediatricians prescribed Benzylpenicillin the most.

The indication of the prescribed antibiotics are categorized into four groups namely empirical treatment,

microbiologically confirmed infection surgical prophylaxis, and medical prophylaxis. Amoxycillin/Clavulanic Acid was most

frequently used in empirical treatment followed by Cefuroxime, Cloxacillin, Ampicillin/Sulbactam and Benzylpenicillin.

Amoxycillin/Clavulanic Acid was most commonly prescribed for microbiologically confirmed infection followed by

Meropenem, Cloxacillin, Ampicillin/Sulbactam and Ciprofloxacin. For surgical prophylaxis, the five most commonly used

antibiotics were Cefuroxime, Amoxycillin/Clavulanic Acid, Ampicillin/Sulbactam, Metronidazole and Cefoperazone. Out of 1059

cases of empirical treatment, 761 (71.86%) cases had the culture sent prior to starting empirical treatment, 265 (25.02%) cases

did not send, and 26 (2.46%) cases with unknown status.

A new programme named Antimicrobial Stewardship Program (AMS) will be implemented in the studied hospitals in

2015 in recognition of its proven value in controlling antibiotic resistance. Antimicrobial stewardship refers to coordinated

interventions designed to improve and measure the appropriate use of antimicrobials by promoting the selection of the

optimal antimicrobial drug regimen, dose, duration of therapy, and route of administration. Once the antimicrobial

stewardship progamme is fully implemented in these hospitals, another Antibiotic Point Prevalence Survey will be conducted in

order to compare the patterns of antibiotic use and the impact of the programme.
