Reactions 1380 - 3 Dec 2011 S Tetrabenazine Quadriparesis and dysarthria (first reports) in a child: case report A 7-year-old girl developed quadriparesis and dysarthria during treatment with tetrabenazine for rheumatic chorea. The girl was admitted with excessive sleepiness for 1 day, weakness of all four limbs for 8 hours, and inability to speak for 4 hours. She had been diagnosed with rheumatic chorea 2 days previously, and had been started on treatment including tetrabenazine 100 mg/day in 3 divided doses [route not stated]. On examination, she was unable to vocalise. In all four limbs, her tone was decreased, and power was 2/5. Cranial nerve examination was normal, abdominal reflex was present, deep tendon reflexes were normal in all limbs and there was no truncal weakness. Her plantars were flexor bilaterally, and there were no involuntary movements or meningeal signs. She had received the last tetrabenazine dose 9 hours earlier. A tetrabenazine adverse event was suspected based on her history and examination findings. One hour after admission, the girl started vocalising. Power gradually increased in all four limbs, and she was able to walk within 18 hours. When involuntary movements reappeared, she refused tetrabenazine, and haloperidol was started.
