What is this product?

This product offers a hospitalisation and surgical takaful

cover for reimbursements of in-hospital treatment or

surgical expenses incurred due to illness and accident,

various outpatient treatment. MediBooster Takaful

provides an optional “top-up” takaful for those who want

higher cover as an additional coverage on a “top-up” basis

and will only pay after the Covered Person has exhausted

all other avenues of compensation from other insurance/

takaful certificates and shall act as a certificate of last resort.

What are the covers or benefits provided?

Takaful product shall only be valid until 30 June 2115 from the

launch date unless cancelled or withdrawn in accordance with the

terms and conditions stipulated in this certificate. The launch date is

21 November 2016 and the product will no longer be valid after 30

June 2115.

The annual renewal contributions payable are not guaranteed. The

Company reserves the right to revise the contribution rate every three

(3) years and the respective revised contribution shall be applicable

at the time of next renewal. Such changes shall be applicable to all

certificate holders irrespective of their claims experiences and shall be

in accordance with the Company’s risk assessment.

All calculations and payments made by the participant under this

Agreement are payable to the respective party in Singapore Dollar

(SGD) currency.

How much contribution do I have to pay?

The contribution you have to pay may vary depending on :

Renewal contribution will automatically be adjusted on entering the

next age group. Contribution may also be adjusted should there be

changes to your occupation at the time of renewal. The contributions

for standard risks are as per the Table of Contribution on page 5, and

are inclusive of the Goods and Services Tax.

What are the fees and charges that I have to pay?

Apakah produk ini?

Produk ini menawarkan perlindungan Takaful untuk kemasukan ke hospital dan pembedahan bagi pembayaran rawatan di hospital atau tangunggan pembedahan akibat penyakit dan kemalangan serta pelbagai rawatan pesakit luar. ‘MediBooster Takaful’ menyediakan pilihan tambahan “top-

up” takaful bagi mereka yang ingin mendapatkan perlindungan

tambahan lebih tinggi yang hanya akan dibayar selepas dana

daripada sijil insurans / takaful lain bagi Orang yang Dilindungi

telah habis dituntut bagi tujuan pampasan dan Sijil ini bertindak

sebagai sijil terakhir.

Apakah perlindungan atau manfaat yang ditawarkan?

Produk ini hanya sah sehingga 30 Jun 2115 dari tarikh pelancaran

melainkan jika dibatalkan atau ditarik balik mengikut terma dan syarat

yang ditetapkan dalam sijil ini. Tarikh pelancaran adalah 21 November 2016 dan produk ini tidak lagi sah selepas 30 Jun 2115.

Tiada jaminan bagi Caruman pembaharuan tahunan yang perlu

dibayar. Semakan semula kadar caruman oleh Pengendali Takaful

akan dilakukan setiap tiga tahun dan caruman selepas semakan

ini akan digunapakai pada masa pembaharuan yang akan datang.

Perubahan tersebut akan digunapakai kepada semua pemegang sijil

tanpa mengira pengalaman tuntutan dan berdasarkan penilaian risiko

Pengendali Takaful.

Semua pengiraan dan pembayaran yang dibuat oleh peserta di bawah

sijil ini akan dibayar kepada pihak yang berkenaan menggunakan mata

wang Dolar Singapura (SGD).

Berapakah jumlah caruman Takaful yang perlu dibayar?

Caruman yang anda perlu bayar bergantung kepada:

Caruman pembaharuan secara automatik akan diselaraskan apabila

umur anda beralih kepada kumpulan umur seterusnya. Caruman

juga akan diselaraskan jika pekerjaan anda berubah pada masa

pembaharuan dilakukan. Caruman untuk risiko biasa adalah seperti

di dalam Jadual Caruman pada mukasurat 5 (termasuk cukai barang

dan perkhidmatan).

What are some of the key terms and conditions that I should be aware of?

• DeductiblePerDisability

This is the amount of expenses you will bear for each disability.

Only the amount exceeding this deductible (and all avenues

of compensation from the basic Takaful certificate to which the

Endorsement is attached) will be payable by this certificate.

• Cooling-OffPeriod

If this certificate shall have been issued and for any reason

whatsoever the covered person shall decide not to take up the

certificate, the covered person may return the certificate to

Archipelago Insurance Limited for cancellation provided such

request for cancellation is delivered by the covered person to

Archipelago Insurance Limited within fifteen (15) days from the

date of delivery of the certificate. The covered person is entitled to

the return of the full contribution paid less deduction of any

expenses incurred by Archipelago Insurance Limited in the issue of

the certificate.

• QualifyingorWaitingPeriod

The eligibility for benefits under the certificate will only start thirty

(30) days after the effective date of the certificate except for

accident. Unless renewed, the coverage will automatically cease

on the expiry date and Archipelago Insurance Limited shall strictly

not be liable for any expense that takes place on the date and

thereafter.

• UpgradedRoomandBoardCo-Payment

You will have to pay 20% of the eligible expenses if you are

hospitalised at a published room and board rate which is higher

than what you are entitled.

• UpgradingCoveredPlan

Application for upgrading of plan can only be made on next

renewal date and is subject to underwriting and acceptance by

Archipelago Insurance Limited.

Can I choose to be treated overseas?

The product provides protection when the certificate:-

• Extended to cover elective treatment in Singapore and Brunei by

paying a contribution loading as imposed by Archipelago

Insurance Limited.

• Whilst you travel or reside overseas for not more than ninety

(90) consecutive days, seek overseas medical treatment when

treatment is not available in Malaysia or upon recommendation of

a panel physician from company panel.

Apakah terma dan syarat utama yang perlu diketahui?

• DeduktibelSetiapHilangUpaya Ini adalah jumlah perbelanjaan yang anda akan tanggung

bagi setiap hilang upaya. Hanya jumlah yang melebihi deduktibel

ini (dan semua pampasan daripada sijil takaful asas dimana

Endosmen dilampirkan) akan dibayar oleh sijil ini.

• TempohBertenang Jika sijil ini telah dikeluarkan dan atas sebarang sebab peserta/

orang yang dilindungi memutuskan untuk tidak menerima sijil

itu / membatalkan, peserta / orang yang dilindungi boleh

mengembalikan sijil tersebut kepada Pengendali Takaful dalam

tempoh lima belas (15 ) hari dari tarikh Sijil diterima. Peserta /

Orang yang dilindungi berhak mendapat pemulangan caruman

yang telah dibayar setelah ditolak dengan perbelanjaan yang

dilakukan oleh Pengendali Takaful sepanjang tempoh pengeluaran

sijil hingga pembatalan sijil terbabit.

• KelayakanatauTempohMenunggu Kelayakan untuk manfaat di bawah sijil ini hanya akan bermula

tiga puluh (30) hari selepas tarikh sijil berkuat kuasa kecuali

kecederaan yang disebabkan kemalangan. Perlindungan

secara automatik akan terhenti pada tarikh tamat tempoh dan

Pengendali Takaful tidak akan bertanggungjawab ke atas apa-

apa perbelanjaan yang berlaku pada tarikh tersebut dan selepas

itu melainkan Sijil diperbaharui.

• MenaikTarafKelayakanBilikHospital&Makanan Anda perlu membayar 20% daripada kos perbelanjaan sekiranya

anda dimasukkan ke hospital dimana kadarnya lebih tinggi

daripada kadar kelayakan anda.

• NaikTarafPelanPerlindungan Permohonan untuk menaik taraf pelan hanya boleh dibuat

pada tarikh pembaharuan seterusnya dan adalah tertakluk kepada

Pengunderaitan oleh penggubal sijil Pengendali Takaful.

Bolehkah saya memilih untuk dirawat di luar negara?

Sijil ini memberi perlindungan semasa:-

• Anda boleh meminta sijil anda untuk dilanjutkan untuk

merangkumi rawatan elektif di Singapura dan Brunei dengan

membayar bebanan caruman yang dikenakan oleh Archipelago

Insurance Limited.

• Ketika di dalam perjalanan atau berada di luar negeri tidak lebih

daripada sembilan puluh (90) hari berturut-turut, mendapatkan

rawatan perubatan di luar negara apabila rawatan tidak boleh

didapati di Malaysia atau atas syor seorang doktor panel daripada

panel Operator Takaful.

What are the major exclusions under this certificate?

• Pre-existing illnesses.• Any medical or physical condition arising within the first 30 days of the covered person’s cover or date of reinstatement whichever is latest except for accidental injuries.• Care or treatment for which payment is not required or to the extent which is payable by any other Takaful or indemnity covering the covered person disabilities arising out of their duties of their employment or profession that are covered under a Workmen’s Compensation Insurance / Takaful Agreement.• Any treatment or surgical operation for congenital abnormalities or deformities including hereditary conditions.• Pregnancy, child birth (including surgical delivery), miscarriage, abortion and prenatal or postnatal care and surgical, mechanical or chemical contraceptive methods of birth control or treatment pertaining to infertility. Erectile dysfunction and tests or treatment related to impotence or sterilization.• Psychotic, mental or nervous disorders (including any neuroses and their physiological or psychosomatic manifestations).• Any event prohibited by Shariah principles.

Can I cancel my certificate?

You may cancel the certificate at any time by giving written notice to us. Upon cancellation, any refund of the contribution would be based on the conditions stipulated in the certificate agreement.

What do I need to do if there are changes to my contact / personal details?

It is important that you inform us of any change in your life profile including your occupation and personal pursuits which would affect the risk profile.

Apakah pengecualian utama di bawah sijil ini?

• Penyakit yang sedia ada.• Sebarang perubatan atau keadaan fizikal yang berlaku terlebih dahulu samada dalam tempoh 30 hari pertama perlindungan atau tarikh pengembalian semula orang yang dilindungi kecuali kecederaan akibat kemalangan.• Jagaan atau rawatan yang pembayarannya tidak diperlukan atau hanya setakat yang dibayar oleh mana-mana indemniti takaful atau lain-lain yang meliputi hilang upaya orang yang dilindungi disebabkan tugas pekerjaan atau kerjaya mereka telah dilindungi di bawah Pampasan Insurans / Takaful Kontrak Pekerja.• Sebarang rawatan atau operasi pembedahan untuk keabnormalan atau kecacatan kongenital termasuk penyakit keturunan.• Kehamilan, melahirkan anak (termasuk kelahiran secara pembedahan), keguguran, menggugurkan kandungan dan pranatal atau postnatum dan pembedahan, mekanikal atau kimia kaedah kawalan kelahiran kontraseptif atau rawatan berkaitan ketidaksuburan. Disfungsi erektil dan ujian atau rawatan berkaitan impoten atau pensterilan.• Psikotik, mental atau gangguan saraf (termasuk sebarang neurosis dan manifestasi fisiologi atau psikosomatik mereka).• Sebarang aktiviti yang dilarang oleh Syariah.

Bolehkah sijil Takaful ini dibatalkan?

Anda boleh membatalkan sijil Takaful pada bila-bila masa dengan memberi notis bertulis kepada kami. Selepas pembatalan, sebarang pemulangan caruman akan berdasarkan kepada syarat-syarat yang ditetapkan dalam sijil.

Who is Archipelago Insurance Limited

Archipelago Insurance Limited (AIL) is licensed under the Labuan Financial Services and Securities Act 2010, regulated by Labuan FSA. The approval for AIL to run its Takaful business (window) has been obtained on 24th April 2015 under the Labuan Islamic Financial Services and Securities Act 2010. Under this Takaful window, AIL has adopted the principle of Wakalah as its business operation model to manage its funds.
