  • In the 2012, The Pusat Pengimejan Diagnostik Nuklear would be two years old, and we are pleased to introduce the 3rd issue of the Annual Book 2012 in celebration.

    It is due to the dedicated efforts by our local researchers and collaborators that our agenda is attracting high quality research works and the effort is becoming more and more crystalised amongst our molecular profession peers.

    This book edition offers a valuable documentation on our achievement across the year of 2012 which highlights examples from actual scientific sessions by us, our fellow collaborators, student held nationally and internationally. In other words, it addresses the exemplary works towards ameliorating research in translational imaging which offers another journey in the personalized medicine era.

    At this point in time, quality becomes a dominant factor of our success in every means and ways, we are leaping forward without exclusion of our constant commitment with the related industry and communities in the provision of education.

    I thank all the contributing efforts by parties for their exceptional works on behalf of PPDN.

  • It is my privilege to welcome you to the insight of the year-round activities and achievement of the Pusat Pengimejan Diagnostik Nuklear, UPM.

    It gives us immense pleasure to see PPDN grows by leaps and bounds, and become stronger with each passing day.

    Efforts will be made to envisage connections to national and international programmes in the field of research and mobility, which will support molecular imaging as the most challenging era in medical imaging.

    This 3rd issue of the annual book highlights multidisciplinary contents, which include continual substantiation of our research activities and hence the yearly research report.

    At the end of this preface, I would like to take the opportunity to extend my sincere acknowledgement to all the collaborators and supporting staffs of PPDN, for extending their kind support and co-operation which truly helped PPDN to grow and become as successful as it is today.

    Mini Symposium on Biotracer Discovery: from bench to bedside23 June 2012

    The symposium was aimed at formulating a common research theme in Molecular Imaging whereby the development and use of positron emission tomography (PET), for in vivo functional imaging of tumours can be utilized for the cutting edge research. Most of the lectures in the event highlighted the benefits of the technology employing SUV (quantitative method) of different tumour biology in human subjects as a method of research in presurgical staging of tumour, treatment monitoring, localization of tumour recurrence , prediction of tumour aggressiveness and prognostication of patient survival. Alongside PET, there was also discussion on the role of other molecular imaging technique employing high resolution quantitative autoradiography study i.e mRNA for preclinical and basic imaging research, with a focus on early clinical translational studies.

    The event was officiated by Prof. Dato’ Ir. Dr. Mohd Saleh Jaafar, the Deputy Vice Chancellor (Research and Innovation) and was attended by more than 50 scientific research collaborators from various biomedical disciplines.

    Achievement: Formation of a translational Research Group in Molecular Imaging, UPM senate has approved a research programme in Molecular imaging in early 2013.

    Annual Scientific Meeting of the College of Radiology30th Mar - 1st Apr 2012The annual Scientific Meeting of the College of Radiology provided one of the most important opportunity in showcasing excellence and innovation in Cancer Imaging. The conference committee had put together a very interesting program that could cater for the different needs of the radiologists and radiographers. The program also provided opportunities to network, exchange information, resources and ideas and to explore new and innovative work. There were series of seminars organised by 5 different Special Interest Groups (SIG) - Head & Neck, Paediatrics, Musculoskeletal, Body Imaging and Interventional Radiology. A free oral paper presentation was also held with 9 papers presented. Associate Professor Dr. Ng Chaan from MD Anderson gave a tutorial for those taking their Fellowship of the Royal College of Radiologists, UK or the Masters (Radiology) Programme, which was well attended by more than 20 trainees. Our fellow researcher, Prof. Dr. Abdul Jalil Nordin bagged in silver medal for his outstanding performance in an oral presentation.

  • National Conference on Physic 2012 (PERFIK), Colmar Tropicale, Bukit Tinggi, Pahang19-21st November 2012

    Malaysian Endocrine Society Annual Congress24th - 27th May 2012

    PERFIK 2012 aimed to promote scientific collaboration and research findings among scientists and industries involved in various aspects of basic and applied physics. The conference was held in Colmar Tropicale, Bukit Tinggi, Pahang between 19th to 21st November 2012. Young researchers, participants from industries, universities, governmental research institutions, postgraduate students and interested parties were presented at the event. There were about 80 papers presented during the oral parallel presentation.

    Achievement: Hishar Hassan, AS Fathinul Fikri, M Salasiah, MN Noramaliza, N Abdul Jalil. Association of total activity 18F-FDG injected with the overall PET image quality in oncology patient: Oral Presentation. The abstract is published in a PERFIK 2012 proceeding book.

    Meanwhile the full paper entitled Impact of total activity variation in 18F-FDG injected with the overall PET image quality in oncology patients has been selected for publication in peered reviewed AIP Conference Proceeding (SCOPUS cited).

    The conference featured several distinguished speakers who had shared their expertise, and perspectives in various areas of mutual relevance. Apart from updating endocrinologists, physicians, trainees and medical officers in the rapidly growing field of endocrinology and metabolic medicine, the Congress had also served as an occasion where collaborative works in various fields of medicine had taken place to enhance scientific knowledge in the field of endocrinology in particular. The congress was aimed at putting a strong emphasis on current topics in the diagnosis and management of endocrine disorders. Molecular imaging in malignant phaechromocytoma –paraganglioma was a theme for a poster presentation represented by our research collaborator- Dr Teik Hin Tan (National Institute of Malaysia) had added in a new perspective the evaluation of such cancer whereby the use of a new 68 Ga-DOTATATE PET/CT was compared with 18F-FDG PET/CT on the overall accuracy performance.

    The Health Seminar, USIM19th-20th May 2012

    Annual Congress of the European Association of Nuclear Medicine , Milan, Italy October 27 – 31, 2012

    The health seminar of Faculty of Medicine and Health Sciences, Universiti Sains Islam Malaysia (FPSK, USIM) was an international scientific event held annually since the year 2006. The aim of the programme was to foster professional knowledge and enhance research in the field of medicine, with Islam as the foundation.

    Attendees to this event were amongst local health professionals, international researchers and medical personnel especially from Islamic countries. USIM’s 1st International Conference on Medicine and Health stimulated new scientific enquiries and academic discourse relating to health issues according to the perspective of the Muslim ummah.

    The European Association of Nuclear Medicine (EANM) is the umbrella organisation of nuclear medicine in Europe and represents the sector towards the European Institutions. Within this role, the EANM aims at advancing science and education in nuclear medicine for the benefit of public health as well as at promoting and co-coordinating, throughout Europe and beyond, discussion and exchange of ideas and results relating to the diagnosis, treatment, research and prevention of diseases through the use of unsealed radioactive substances and the properties of stable nuclides in medicine.

    The goal of the EANM is thus to provide a suitable medium for the dissemination and discussion of the latest results in the field of nuclear medicine and related subjects. Prof. Dr. Abdul jalil Nordin had successfully presented four scientific posters in the field of molecular imaging.

    12th Annual Teaching Course and International cancer imaging Society Meeting, Oxford, UK4th-6th October 2012

    35 participants representing 77 Member States and 16 organizations attended the Conference, which was held at the World Conference Center, Bonn (the former federal government Plenary Chamber).

    Prof. Dr. Abdul Jalil Nordin attended the meeting as a mission to promote and expand the molecular imaging agenda pioneered by PPDN in Malaysia.

    Solving problems in cancer patientsThe twelfth annual teaching course had focused on ‘solving problems in cancer patients’. Content had structured around clinical and technical stumbling blocks met in everyday practice and offer solutions to these issues.

    The 68th Korean Congress of Radiology18th-20th October 2012

    KSR opened its Congress to the international audience in 2007, and since then the number of sessions operated in English has increased to over 70% of the whole program. The conference had designed a program with high quality scientific sessions for which it had successfully provided a great opportunity for all participants to keep up-to-date with the latest research findings. The conference had featured delegates of more than twenty world renowned scholars from 12 countries. Among them, Dr. Gabriel P. Krestin, who is the current president of ESR, will give the Congress Lecture.

    Programme: Molecular Imaging (hWB 07)Synopsis

    In the past decades, we have witnessed the importance of non communicable diseases including coronary artery diseases, malignancy and cerebro-vascular diseases as the main cause of death

    in Malaysian hospitals. Molecular imaging is a new field in medicine characterizing, and quantifying the biological processes at the cellular and subcellular levels within intact living organisms. The field exploits specific molecular probes as the source of image contrast where it provides the

    imaging signal in almost all molecular imaging assays. The development, validation, and application of this novel imaging technique in living subjects should further enhance our understanding of

    disease mechanisms and go hand in hand with targeted personalised therapy. The findings can be correlated with gene expressions and the discovery of novel biomarkers with potential utilization in

    drug discovery.

    Synopsis:Somatic mutations of the EGFR gene are valuable biomarkers for NSCLC patients. It has been reported that EGFR mutations were more frequently detected in selected sub-population of patients with the following characteristics – female gender, adenocarcinoma histology, Asian origin and non-smokers. Better clinical responses to tyrosine kinase inhibitors (TKIs) were observed in this particular group of patients compared to its counterpart. Thus, EGFR mutation testing is now a standard in the management of advanced lung adenocarcinoma in many of the western countries and in some countries and medical centers in East Asia. In Malaysia, lung cancer is the second most common type of cancer among men and ranked sixth among women. As a multi-racial Asian country, Malaysia is comprised of various ethnics. Therefore, we aim to detect major EGFR mutations in FFPE specimens and examine the associations between the mutations and ethnicity, patient’s gender, age at diagnosis, tumor histology, clinical stage, and smoking history.

    Molecular Imaging (Oncology)

    Synopsis:Pheochromocytomas are also referred to as adrenal paragangliomas. Paragangliomas originate from the neural crest-derived paraganglia of the autonomic nervous system. These tumors originate from catecholamine-producing chromaffin cells. The increased glucose uptake imaged with Flurodeoxyglucose Positron Emission Tomography (FDG PET) is largely dependent on the rate of glycolysis. FDG uptake and trapping occurs because of upregulation of glucose transporters (notably GLUT1 and GLUT3) and HEXOKINASES I and II . FDG PET imaging also allows quantification of glucose uptake. It is known that in many studies, poor disease prognosis is consistently correlated with increased tumour inherent increased glucose uptake in tumours

  • Biomedical Science (Cyclotron, Radionuclides Production & Radiopharmaceuticals)

    Synopsis:In view of increasing significance of radiopharmaceuticals in medical diagnosis, niche area of cyclotron, radionuclides production and radiopharmaceuticals possesses an important element in nuclear medicine diagnosis. The physician, specifically the oncologists are no longer preferred the conventional ubiquitous radiopharmaceutical 18F-FDG. New radiopharmaceuticals emerge one after another that are able to provide high sensitivity and specificity when approaching a very specific oncology type disease. Hence, the ability to explore new radiopharmaceuticals either from charged-particle accelerator (cyclotron) or generator, 18F-fluorine based or non- 18F-fluorine based is essential in assisting management of nuclear medicine diagnosis.

    Synopsis:PET CT is a combined multi-modality imaging that provides anatomical locations and functional imaging. The commonly radionuclide used is 18F-FDG ~short half-life (110min). Radioisotope consists of unstable nucleus. Therefore, it will decay to achieve stable configuration of proton and neutron. During radioactive decay, an unstable nucleus spontaneosly decomposes to form a different nucleus, giving off radiation in the form of atomic partices or high energy rays. This decay occurs at a constant, predictable rate that is referred to as half-life.

    Science Physic

    Synopsis:To evaluate the effect of intravenous contrast media during integrated Positron Emission Tomography Computed Tomography (PET/CT) using fluorine-18 fluorodeoxyglucose (18F-FDG) as tracer in cancer imaging.

    This study demonstrates that application of contrast-enhanced CT in PET/CT imaging of cancer patient will cause significant changes on CT values but no significant changes on SUVmax values.

    Among the 191 case-mix referrals, the major indication for referrals are for Rb-82 cardiac perfusion (94), oncology (84), MyoPET (6), Neurology (5) and cardiology (46%) followed by infection ( 2),(Fig 1).The number of the Rb-82 cases has shown a staggering rise above the oncology which is attributable to the recruitment of patient under the approved RUGS grant scheme in 2012.
