    GUIDELINE FOR REPORT WRITING

    A. Noise Monitoring

The Factories and Machinery (Noise Exposure) Regulations 1989 requires factory owner to conduct initial exposure monitoring. The purpose is to determine whether any of his employees are exposed to noise greater than the action level of 85 dB(A). Since this is a preliminary requirement to determine further compliance with the employee exposure monitoring requirements, competent person are reminded to conduct the monitoring in all aspect with high degree of professionalism. Deviation from such ethical practice may result in the factory owner taking wrong course of action which may lead to unwanted repercussion in the future. To avoid such incident, competent persons must be thorough in categorizing employees in the various occupational groupings and carefully selecting the high risk employee to represent the exposure for the group. The instrument used must at all time meet the specified level of accuracy while during monitoring adequate sampling time must be taken to minimize the spasmodic variation in noise level. A repeat of the monitoring should be carried out if the competent person feels that the results obtained are not representative.

    B. TITLE PAGE - Type of monitoring (Initial / Positive / Additional Monitoring) - Name, address and JKKP registration number of factory - Name and registration number of competent person - Date of monitoring

    C. CONTENT i) Introduction

    - Particular of the factory : * Name, address and JKKP registration number

    - Date of monitoring

    - Name and registration number of competent person

    - The purpose of the monitoring

iv) Instrumentation - List down the noise measuring instrument and calibrator used, serial

number and approval number of the instrument and calibrator

    v) Methodology- Briefly describe procedure for :

    a. Area Monitoring - Noise mapping (zoning / contouring)* - Type of noise (continuous, intermittent, impulse, etc)

    b. Personal Monitoring - Classification of all employees in the factory according to job

classification - Selection of employees for monitoring and justification for

selection - Number of employees monitored and justification

(Refer Statistical Table, if necessary) - Sampling duration - Working hours and number of shift - Type of exposure (continuous, intermittent, impulse, etc) - Justification of sampling duration (depends on working hours

    and type of exposure)**

    and peak level

viii) Recommendation* - Based on the result, recommend on parts of the regulation that have

to be complied and type of control measures- Recommendations should be specific (eg specify the working area,

    group of employees)

    ix) Conclusion

    x) Signature, Name and Approval Number of the Competent Person

    xi) Appendix

    a. Process flowchart b. Layout plan for machinery and employees c. Noise mapping d. Data sheet and calculation e. Computer Print-out f. A latest copy of calibration certificate for measuring instrument

    * Noise mapping, discussion and recommendation should be done based on action level and PEL for the working duration.

** eg If type of noise exposure is continuous, and working duration is 8 hours therefore, sampling duration can be 4 6 hours. If type of noise exposure is fluctuating, and working duration is 12 hours therefore, sampling duration must be full period.
