TRAINER PROFILE - Shamsul Rizal Shamsudin is a knowledgeable SHE practitioner, having 15 years’ experience in OSH & EMS (Environment Management System), currently holds a Diploma in Health & Safety from the University Of Perth, Western Australia and in the progress of completing his Master in Occupational Safety & Health. He is a Safety & Health Officer (Green Book holder), Lead Auditor for OHSAS, EMS & ISO9001 as well as County HSE Manager of HSE department at a Multi National Company. Apart from his duty, he is also the Chairman of ESH Sub Committee for FMM Johor Branch & Honorary Member for Southern OSH Practitioner Association. PAYMENT Registration form must be completed and returned to FMM by September 4, 2017 with correct payment in cheque or bank draft made in favour of “Federation of Malaysian Manufacturers” crossed “Account Payee Only”. Non-FMM members MUST PAY the full fee before or on the day of the event.
REFUND POLICY Cancellation must be in writing to FMM. If cancellation is received: More than 14 days prior to event - full refund. Between 8 – 14 days - 50% refund. Less than 7 days of event - no refund.
REPLACEMENT/ CANCELLATION Replacement at no additional cost. FMM reserves the right to cancel or reschedule the programme. All efforts will be taken to inform participants of any changes. If FMM decides to cancel or postpone the seminar, FMM is not responsible for covering airfare, hotel or other travel costs incurred by participants.
