LETTER OF APPOINTMENT
This is to confirm our offer of employment to as _________________________ effective
________________ under the followings terms and conditions.
1. SALARY
Your basic salary will be RM_____________. The annual salary review will be in
___________. As salary is highly confidential matter, you are not allowed to discuss it
with any other person(s), whether they are employees of the Company or not, except with
the Management or Representative(s) authorised by the Management.
2. EPF BENEFITS
You are entitled to statutory EPF benefits.
3. PROBATIONARY PERIOD
You will serve a probationary of three (3) months with effect from date of
commencement. The probationary period may be extended or shortened at the discretion
of the Management and in either case, you will be informed in writing prior to the expiry
of the said three months.
4. TERMINATION OF SERVICE
During your probationary period either you or the Company may terminate the
employment by giving fourteen (14) days notice in writing or fourteen (14) days in lieu
of notice without assigning any reason whatsoever.
Upon confirmation, either you or the Company may terminate the employment by giving
forty-five (45) days notice in writing or forty-five (45) days’ basic salary in lieu of notice.
The company reserve an absolute right to terminate the services of any employee without
notice for fundamental breach of contract or where the employee are suspected to be
guilty of improper behaviour, intentional wrong doing, deliberate violation of rule of
standard of behaviour or such act or omission inconsistent with company rules and policy
whether express or implied.
5. ANNUAL LEAVE
As a confirmed employee, you are entitled to ten (10) working days of annual leave for
the first year of service. For the subsequent continuous year of service, please refer to
handbook, paragraph................ Your leave applications are subject to approval by the
Management. You are only eligible for the paid annual leaves after you have received the
confirmation of your job.
Annual leave accumulated at the time of termination my not be used to offset the
notification period unless otherwise agreed by the Management.
6. GAZETTED PUBLIC HOLIDAY
a) New Year’s Day 1 day
b) Chinese New Year 2 days
c) Hari Raya Puasa 2 days
d) Hari Wilayah Persekutuan 1 day
e) Labour Day 1 day
f) Hari Raya Haji 1 day
g) Birthday of The Yang Dipertuan Agung 1 day
h) Birthday of Prophet Muhammad 1 day
i) National Day 1 day
j) Deepavali 1 day
k) Christmas Day 1 day
7. MEDICAL LEAVE
Years of Service Sick Leave Entitlement Hospitalisation Leave
Less than two (2) years 14 days 46 days
More than two (2) years and 18 days 42 days
Less than five (5) years
More than five (5) years 22 days 38 days
An employee is entitled to a maximum of sixty (60) days hospitalization leave in one
calendar year, which is inclusive of any paid sick leave for that year. The entitlement for
sick leave should not be more than the entitlement as stated above. Sick leave and
hospitalization leave stated should be from the panel doctor / hospital appointed by our
Company.
8. WORKING HOURS
You are adhere to the working hour stipulated in accordance to the work schedule
Monday to Friday : 9.00 a.m to 5.00 p.m
Saturday : 9.00 a.m to 1.00 p.m
9. EMPLOYEE RESPONSIBILITY AND BENEFITS
Your responsibilities and benefits as an employee are documented in the Company
Policies in greater details. The terms and conditions in this letter are extracted from the
Company Policies which prevail over any other documents.
All terms and condition as specified above are subjected to change by the Management
from time to time.
10. OTHERS MATTERS
You are required to carry out any assigned duties instructed by Management or its
Representative from time to time.
If you are agreeable to the above terms and conditions, please sign on both copies of this
letter and return one copy to us.

PER: PERLANTIKAN JAWATAN
Merujuk kepada perkara di atas, sukacita dimaklumkan bahawa pihak syarikat dengan ini
melantik anda sebagai berkuatkuasa pada 2010.
Perlantikan ini adalah berdasarkan kepada syarat dan peraturan di bawah:-
1.0 GAJI
1.0 Gaji Pokok : RM1,000.00/setiap bulan
1.2 Elaun : RM 200.00/setiap bulan (berdasarkan prestasi
/setiap bulan)
1.3 Kemudahan insuran untuk hospital dan kemalangan, selepas disahkan
dalam jawatan.
1.4 Gaji yang ditawarkan adalah sulit dan tidak dibenarkan sampai ke
pengetahuan pekerja lain.
1.5 Pembayaran gaji akan dibayar 2 kali dalam sebulan (10hb dan 20 hb)
1.6 Jika anda ingin pembayaran gaji dibayar sekali setiap akhir bulan, anda
boleh membuat permohonan secara bertulis.
1.7 Pembayaran adalah secara tunai atau cek.
2.0 WAKTU KERJA
2.1 Pekerja dikehendaki mematuhi waktu kerja yang ditetapkan seperti
berikut:
Bahagian Pengurusan : 9 jam/hari (fleksi)
Kafeteria Hospital : 2 syif
Restoran : 2 syif

3.0 TEMPOH PERCUBAAN
3.1 Pihak syarikat akan memberi 3 bulan tempoh percubaan, perlantikan ke
jawatan tetap adalah dinilai dari prestasi kerja anda.
3.2 Jika pihak syarikat mendapati prestasi kerja anda tidak memuaskan, pihak
syarikat berhak melanjutkan tempoh percubaan atau menamatkan
perkhidmatan anda dalam tempoh 24 jam.
4.0 NOTIS BERHENTI KERJA
4.1 Semasa dalam tempoh percubaan, tempoh notis berhenti kerja adalah
selama 24 jam.
4.2 Setelah disahkan dalam jawatan, notis berhenti kerja adalah selama 30 hari
atau 30 hari gaji sebagai ganti notis.
5.0 KERJA LEBIH MASA
Pekerja berhak untuk membuat tuntutan elaun kerja lebih masa mengikut bidang
tugas dan kelulusan daripada Ketua.
6.0 CUTI TAHUNAN
Pekerja berhak untuk mendapat cuti tahunan setelah disahkan dalam jawatan.
Pemberian cuti tahunan adalah seperti berikut:-
Kurang dari 2 tahun : 14 hari
2 tahun dan kurang dari 5 tahun : 18 hari
5 tahun atau lebih : 16 hari
7.0 CUTI SAKIT
7.1 Pekerja layak untuk mendapat cuti sakit berbayar setelah disahkan dalam
jawatan. Pemberian cuti sakit berbayar adalah seperti berikut:
Kurang dari 2 tahun : 14 hari
2 tahun dan kurang dari 5 tahun : 18 hari
5 tahun atau lebih : 22 hari
7.2 Jika dimasukkan ke hospital atau cuti bersalin, pekerja berhak mendapat
cuti maksima 60 hari dalam setahun. Cuti ini tidak termasuk dalam cuti
sakit tahunan.

7.3 Pihak syarikat hanya akan menanggung kos rawatan perubatan biasa
(tidak termasuk pembedahan dan pergigian) setelah disahkan jawatan dan
jika dalam tempoh percubaan kos perubatan ini adalah di dalam
tanggungan sendiri.
8.0 CUTI UMUM
Senarai cuti umum di mana syarikat akan bercuti, adalah seperti berikut:-
1) Tahun Baru
2) Tahun Baru Cina
3) Hari Raya Puasa
4) Hari Wilayah Persekutuan
5) Hari Pekerja
6) Hari Raya Haji
7) Hari Keputeraan YDP Agong
8) Maulidur Rasul
9) Awal Muharram
10) Hari Kemerdekaan
11) Hari Raya Depavali
12) Hari Raya Natal
• Sebarang perubahan akan diberitahu dari semasa ke semasa
9.0 PERATURAN SYARIKAT
Pekerja dikehendaki mematuhi setiap peraturan syarikat yang telah ditetapkan dan
pihak syarikat berhak menamatkan perkhidmatan jika terdapat pekerja yang
melanggar peraturan yang telah ditetapkan.
Sekiranya encik/cik/puan bersetuju dengan tawaran dan syarat-syarat yang telah
dinyatakan, sila tandatangan surat ini dan kembalikan kepada Jabatan Sumber Manusia &
Pentadbiran.
