Teknik-Teknik Diagnosis Penyakit Malaria Patricia Lim Kim Chooi

Malaria masih merupakan salah satu di antara penyakit-penyakit penting yang dihidapi manusia. Di Malaysia, penyakit malaria terus menjadi satu rnasalah kesihatan tnasyarakat yang mengakibatkan morbiditi dan kadang-kadang mortaliti. Plas~~zodiz~m,falczpa~~z~?n adalah parasit yang terpenting.

Aspek yang penting untuk pengurusan dan pengawalan penyakit malaria ialah untuk membuat diagnosis infeksi ini secara cepat dan tepat. Kebolehan untuk mengesan parasit malaria di dalam manusia adalah prosedur penting untuk kedua-dua diagnosis individu dan juga kajian epidemiologi. Pemeriksaan mikroskopi filem darah untuk mengesan parasit malaria merupakan kaedah pengesanan parasit yang utama. Iiaedah ini adalah spesifik dan sensitif kerana dapat mengesan parasitemia kurang daripada 0.00019h (Bruce-Chwatt, 19841, tetapi juruteknik yang terlatih diperlukan bagi memeriksa setiap smear darah. Ia juga memakan masa, mahal dan tidak begi'tu sesuai untuk kajian epidemiologi yang besar.

Perkembangan terbaru dalam diagnosis parasit malaria ialah teknik quantitatif buffy coat (QBC) untuk ujian pantas bagi parasit malaria melalui pemerhatian langsung sampel darah yang telah dicentrifuge di dalam tiub QBC (Paray et 01, 1990). Ujian QBC dapat mengesan paras parasitemia serendah 0.00005%. Kajian oleh IPP bagaimanapun menunjukkan yang ujian ini kurang superior daripada kaedah filem darah. Tarnbahan lagi, ujian ini tidak boleh mengkuantitat parasitemia secara mudah dan adalah mahal untuk dikendalikan bagi kajian epidemiologi pang besar.

Selama tempoh beberapa tahun sebilangan ujian serr>lngi telah dihcnti~k hagi mengeyan samada antibodi malaria atau antigen plasmodium di dalam sera pesakit-pesakit malaria. Asai serologi bagi mengesan antibodi adalah lebih berguna

sebagai alat epidemiologikal untuk mengukur dan rnengawasi endemisiti malaria, tahap transmisi malaria, kesan kaedah kawalan lebih daripada untuk diagnosis individu kerana kehadiran antibodi dalam serum menggambarkan infeksi masa lalu dan juga masa kini.

Ujian gel precipitasi secara tekniknya adalah mudah dan tidak mahal, tetapi telah dibuktikan secara relatifnya tidak sensitif d a n juga memerlukan kepekatan antigen yang tinggi. Ujian 'indirect haemagglutinarion' CIHA) adalah di anlard ujian-ujian terawal dibentuk bagi mengesan antibodi malaria. Ujian ini mudah dan tidak mahal serta jumlah sampel yang besar boleh diproses dalam satu masa . Walau bagaimanapun, ia kurang sensitif dalam mengesan infeksi awal.

Ujian indirect fluorescent antibndi (TFAT) telsh dalam tahun-tahun kebelakangan ini merupakan ujian yang paling diharapkan untuk mengesan antibodi kerana sensitiviti dan spesifikasinya. Kegunaannya sebagai a l a ~ cpidcn~iulugikal telah ditunjukkan di Malaysia iMak et al, 1987). Ujian ini telah dibuktikan berguna sebagai petunjuk untuk pengalaman malaria dalam populasi, tahap endemisiti dan transmisi, dan juga pengawasan kaedah kawalan untuk malaria. Ujian ini memerlukan mikroskop fluoresen, ini men- jadikanny:2 tidak praktikal untuk kegunaan di lapangan, dan juga pembacaan slaid oleh individu- individu adalah subjektif. Dengan pembentukan asai 'enzyme-linked immunosobent' (ELISA), kcputusail ujian buleh dilihat dengan mata kasar dan ini menjadikannya berguna di lapangan. Ia juga menghasilkan keputusan yang objektif dan boleh disepara automasikan dan dipiawaikan bagi menguji jumlah sampel yang besar dalam satu masa (Bidwwell and Voller, 1981).

Pendekatan yang alternatif untuk mikroskop cahaya bagi diagnosis kehadiran plasmodia dalam darah manusia ialah pengesanan antigen parasit atau asid deoxyribonucleic (DNA) parasit.

Pengenalan bioteknologi dalam tahun-tahun kebelakangan ini telah membolehkan penggunaan antibodi monoklonal dan probe DNA Jalarn diagnosis malaria.

Teknologi 'recombinant DNA' telah membawa kepada pembentukan probe DNA bagi P. falcipariunz dan P. ~&mh: untuk diagnosis infeksi manusia dan vektor nyamuk. Tahap parasitemia yang bolch dikcsan olch probe-probe ini ialah lebih kurang 0.0001%, iaitu bersamaan dengan sensitiviti pemeriksaan filem darah. Pendekatan lain pula rnenggunakan probe yang spesifik untuk species untuk mengesan RNA ribosom (rRNA) parasit malaria oleh kerana probe yang mengesan sasaran ini berkemungkinan lebih scnsitif kerana bilangan salinan scqucncc sasaran lebih tinggi dan heterogenous. Bagaimanapun, kaedah ini belum lagi diuji untuk sarnpel dari pesakit-pesakit, dan soalan tentang kestabilan rRNA sasaran untuk sanlpel-sampel di lapangan perlu dijelaskan. Potensi penggunaan probe- probe DNA sebagai altenatif kepada pemeriksaan filem darah memberiksln beberapa kclcbihan terutanlanya untuk tinjauan yang besar ke atas penduduk di kawasan bermalaria. Pada masa kini, kebanyakan probe DNA memerlukan label radioaktif dan ini menjadikannya tidak praktikal sama ada untuk kegunaan klinikal atau di lapangan. Justeru itu adalah perlu untuk mencari kaedah untuk melabel probe DNA bukan menggunakan bahan radioaktif. Pada masa sekarang kajian sedang dijalankan di IPP bagi membentuk probe DNA dengan label bukan radioaktif bagi parasit malaria daripada manusia dan monyet untuk digunakan sebagai alat diagnosis dan seroepidemiologi.

Perkembangan penting yang lain ialah pengenalan teknik hibridoma untuk penghasilan antibodi monoklonal. Teknik ini sudah digunakan secara meluas bagi menghasilkan antibodi rnonoklonal terhadap berbagai-bagai peringkat hidup parasit malaria untuk mengenali parasit, diagnosis dan kajian-kajian imunologi. Antibodi monoklonal terhadap protein circumsporozoit parasit malaria telah digunakan untuk menentukan prevalen nyamuk yang terinfeksi, beban sporozoitnya dan status species sporozoit yang menginfeksi.

Antibodi monoklonal yang dihasilkan terhadap peringkat aseksual dalam darah dengan spesifisiti yarlg jclas akan bergurla untuk diagnosis antigen plamodium dalam serum penyakit malaria. Antibodi monoklonal yang begitu telah digunakan dalam ELISA dan asai imunoradiometri (IRMA) untuk mengesan antigen kedua-dua asai ini menunjukkkan sensitiviti yang baik dan IRMA menujukkan korelasi yang baik dengan parasitemia dengan kebolehan untuk mcngcsan parasit malaria serendah 2.4 parasit bagi setiap lo8 sel darah merah. Antibodi monoklonal yang dibentuk terhadap P. ji;llcipawm dan P. cynomolgi di IPP juga telah menunjukkan spesifisiti dan sensitiviti yang baik dalam 'sandwich-ELISA' untuk mengesan antigen (Lim et a l , 1992). Tugas se terusnya un tuk mcmbcn tuk asai 'do t immunobinding' menggunakan monoklonal- monoklonal tersebut akan membolehkan pembentukan ujian lapangan yang pantas untuk kajian seroepidemiologi.

Cryptosporidium adalah protozoa coccidia yang biasa terdapat pada haiwan vertebrata. Spesies- spesies C'yptosporidium adalah berkaitan diantara satu sama lain tetapi Cryptosporidium pamum adalah hanya satu spesies yang menyebabkan penyakit pada manusia. Infeksi ini adalah zoonotik dan haiwan seperti reptilia, burung, kuda, anak lembu, babi dan kucing bertindak sebagai hos perumah. Jangkitan protozoa ini adalah melalui najis-ke-mulut atau melalui kontaminasi air minuman. Organisma ini tidak dapat dimatikan ole11 klorin dan tidak tertapis dengan menggunakan penapisan air biasa untuk bekalan air ke rumah-rumah.

Organisma ini boleh menjadi komensal atau pengambil peluang (opportunistic) dan ia boleh bertindak sebagai benigna atau palogen yang mengancam kehidupan bergantung kepada status immuniti individu itu. Kadar pembawa yang tinggi didapati di kalangan pesakit yang tiada gejala (asimptomatik) yang menjalani pemeriksaan gastroduodenoskopi untuk penyakit-penyakit yang lain.

Cyptosporidium biasanya terdapat pada kanak- kanak terutamanya dari golongan sosioekonomi yang rendah. Cyptosporidium menyebabkan cirit- hirit yang tenik dikalangan kanak-kanak yang mengalami hipogammaglobuline~nia dan yang k e k u r a n g a n daya p e r t a h a n a n t u b u h (immunodificient) serta pesakit-pesakit AIDS. Ia juga biasa bwja~~gk i t kepada pengembara- pengembara yang pergi ke negara-negara yang kurang maju. Beberapa kawasan di USA dan England dilaporkan sebagai kawasan epidemik bagi Cyptosporidium. Transmisi di sini adalah disebabkan oleh kontaminasi air minuman dengan najis binatang.

Kepentingan Cryptosporidium sebaga~ agen patogen hanya dikenalpasti sejak 8 tahun kebelakangan ini disebabkan oleh kepekaan

terhadap penyakit ini dan pembaharuan di dalam teknik pendiagnosaan. Penemuan terbaru melaporkan Cpptospomdzum boleh menyebabkan sindrom kolestatik yang disebabkan oleh kolangitis sklerosis dan papilari sterosis terutamanya pada pesakit AIDS.

Bagaimanapun pengetahuan tentang infeksi ini sangat kurang di Malaysia. Pada 1987, satu kajian di kalangan pesakit-pesakit pediatrik di wad gastroenteritis, Hospital Klang menunjukkan kekerapan Cyptosporidium ialah 4943. Kajian lain dibuat di Hospital Universiti menunjukkan kckcrapannya ialah 2.1%. Kajian di Kelantan pula menunjukan 11.5% adalah positif di kalangan kanak-kanak yang berumur 7 tahun ke bawah yang dimasukkan ke hospital di sebabkan oleh cirit-birit. UI dalam komuniti di Kelantan, dikalangan kanak-kanak yang mengalami cirit- birit, 10.6% dari contoh najis yang diperiksa didapati mengandungi infeksi Cryptosporidium. Kemungkinan organisma ini penting sebagai agen aetiologikal yang menyebabkan cirit-birit di kalangan kanak-kanak di negara ini. Di negara ini juga tiada angka yang menunjukkan kekerapan akibat dari infeksi ini di kalangan pesakit yang mempunyai sindrom kurang daya tahan tetapi kajian turut dibuat bagi kes-kes yang positif.

Secara umum, tiada kemoterapi atau i m u n o t e r a p i y a n g spes i f ik t e r h a d a p Cyptosporidium yang telah dikenal pasti tetapi ubat-ubat tertentu seperti Octeotide laitu analog somatostatin, Zidovudine, Halofuginone lactate, Spiramicin, Azithromycin serta sebahagian Sulphonamides seperti Sulfadimethoxin dan Sulfamethadine, telah memberikan hasil yang menggalakan. Dalam keadaan biasa gejala-gejala penyakit akan pulih seperti biasa dalam beberapa hari s a h j a tetapi di kalangan mcrcka yang mempunyai immune sistern yang tidak normal, gejala-gejala penyakit ini boleh berlarutan.

Disebabkan kepentingannya sebagai penyebab yang kurang daya tahan. Teknik pencelupan cirit-birit baru sahaja di temui maka kajian Ziehl-Neelsen yang diubahsuai biasa digunakan t e r h a d a ~ n ~ a belum dibuat secara menyeluruh ,,tuk menjadikan organisma ini rnerah pad2 di makmal-makmal'di Malaysia. Memandangkan smer najis dan menjadikannya nludah dilihat di ubat yang berkesan mungkin didapati tidak lamalagi ,diagnosamakn~alberkaitandengan b a w a h m i k r o s k o p k a m p a u n . T e k n i k organisma ini perlu dilakukan terhadap semua immunofloresens dengan menggunakan slat kes cirit-birit di kalangan kanak-kanak dan pes.&t kelengkapan ujian berbentuk kit boleh diperolehi.

Satu Teknologi Imunoasai Baru, MEIA Joseph B Lopez

Ekoran kerja-kerja permulaan Yallow dan Berson pada tahun 19601, teknik imunoasai telah terus berkembang dengan pesat. h i telah menghasilkan berbagai jenis variasi dari teknik asal bagi kerja- kerja mengukur sesuatu bahan analit pada takat kepekatan yang terendah di dalam ceca i r biologikal . Imunoasa i te rdahulu yang menggunakan label radioisotop (oleh itu dikenali sebagai radioimunoasai, RIA) adalah amat sukar untuk dijalankan dan juga imunoasal in1 selalu memberi keputusan yang tidak tepat. Kemajuan berikutnya pula membawa kepada peningkatan ketepatan imunoasai dengan masa ujian yang lebih s ingkat . Tetapi ianya masih te tap menggunakan radioisotop yang mempunyai potensi berbahaya.

Peristiwa penting yang berlaku pada awal tujuh puluhan bagi teknik ini ialah penggunaan enzim bagi menggantikan radioisotop. Ini adalah h a d gagasan penyelidik-penyelidik Sweden iaitu Engvall dan Perlmann',3. Penggunaan enzim menjadi lebih menarik memandangkan ianya lebi l~ selama~, ~cagerl-reagen yang digunakan mempunyai separuh hayat yang lebih panjang dan tidak memerlukan alatan yang mahal untuk mengesan hasil akhir ujian. Enzim alkalin fosfatase dan horseradish peroksidase kerap digunakan untuk teknik enzim - imunoasai (EIA) ini.

Iicperluan untuk menjalankau uji.d11 ke alas bilangan sampel yang banyak dalam masa yang singkat pula telah menghasilkan imunoasai secara automasi terutamanya bagi teknik EIA. Dalam tahun tujuh puluhan, sejenis teknik yang serupa iaitu teknik Enzim Multiplied Imunoasai (EMIT, Syva. Corp., USA) bagi bidang Kimia Klinikal tclah dihasilkan. Ia merupakan satu kejayaau dalam bidang ini walaupun ianya hanya terhad kepada kerja-kerja mengukur molekul-molekul

kecil seperti dadah. Penghujung tahun lapan puluhan pula mempamerkan perkembangan canggih teknik EIA berautomasi penuh yang menggunakan instrumentasi pada keseluruhan perjalanan assei. Salah satu darinya ialah mikropartikcl cnzim imunoasai (MEIA)*.

Teknologi MEIA menggunakan mikropartikel submikron yang diselaputi molekul tertangkap (antigen, antibodi atau partikel virus) yang khusus bagi bahan yang hendak diukur. Kawasan permukaan yang luas dan jarak difusi yang kecil antara bahan analit dan fasa pejal telah menghasilkan peningkatan assei kinetik dan penyingkatan masa inkubasi. Ini membolehkan ujian dapat dijalankan dcngan pantas.

Selepas inkubasi, campuran reaksi antara partikel- partikel mikro dan bahan analit dipindahkan ke dalam matrik fiber kaca lengai di mana partikel mikro akan bercantum secara tak berbalik. Komplek imuno ini akan terlekat pada fiber- fiber kaca manakala bendalir-bendalir reaksi disalur melalui rongga-rongga yang terdapat di dalam matrik. Komplek imuno pada rnatrik fiber kaca bolch dikcsan dcngan pcnggunaan konjugat yang telah dilabel dengan enzim alkalin fosfatase. Enzim ini boleh katalis reaksi hidrolisi 4- methylumbelliferone. Kadar penghasilan 4- methylumbelliferone, iaitu sejenis hasil pendafluor, adalah sekadar kepada kepekatan bahan analit di dalam sampel yang diuji.

Alat-alat yang digunakan untuk sistem assei ini ialah dari Abbott IMX (Abbot Diagnostics, N Chicago, USA). Sistem ini bolch digunakan untuk mengukur bahan analit seperti peptid dan hormon steroid, petanda-petanda tumor, protin lain dan

partikel-partikel virus serta antibodi. Alat ini mudah digunakan, mempunyai automasi penuh dan tidak memerlukan tenaga manusia melainkan proses untuk mernasukkan sampel. Essei IMX boleh menjalankan ujikaji sebanyak 24 sampel pesakit-pesakit secara serentak dan mengambil masa antara 35-45 minit bergantung kepada jenis bahan analit. Manakala ujian RIA dan EIA yang dijalankan secara manual mengambil masa antara beberapa jam hingga 3 hari bagi tiap-tiap kumpulan yang diuji.

Kemudahan Ujian H N di Makmal Rujukan AIDS Kebangsaan Randel Fang

Kemudahan-kemudahan bagi pendiagnosan Human Immunodeficiency Virus (HIV) telah ditubuhkan di Bahagian Virologi, Institut Penyelidlkan Perubatan (IPP) dalam tahun 1985. Sejak itu, peranan unit ini telah diperluaskan dan diperkembangkan. Dalam tahun 1987, Kementerian Kesihatan Malaysia (KKM) telah mengiktirafkan IPP sebagai Makmal Rujukan AIDS Kebangsaan (NARL) dan Makmal HIV merupakan salah satu komponen utama kemudahan baru ini.

Kemudahan-kemudahan yang ditawarkan oleh NARL bagi diagnosa HIV boleh dibahagikan ke dalam 3 kategori umum: saringan, pengesahan dan penyelidikan. NARL berkhidmat sebagai salah satu pusat saringan bagi pihak KKM. Walau bagaimnnnpun, in berlainnn dnri pusat-pusat saringan yang lain di mana ujian diagnosa HIV yang dijalankan oleh NARL tidak terhad kepada ujian saringan yang telah dipilih sahaja. Buat masa kini, NARL secara rutin menggunakan dua jenis ujian saringan iaitu, teknik ujian recombinant competitive enzyme linked immunosorbent dan ujian agglutination menggunakan partikel sintetik. Protokol ini sentiasa diulangkaji dan dikemaskini. Selain dari kedua-dua ujian ini, NARL juga terlibat dalam penilaian kit-kit/protokol-protokol saringan yang baru dan salah satu fungsi NARL ialah menilai Kit diagnosa HIV baru sebelum ia diperkenalkan ke pasaran tempatan.

Selain dari menjalankan ujian-ujian saringan HIV, NARL juga berfungsi sebagai makmal yang tunggal di negara ini yang telah diarahkan untuk mcnjalankan ujian pengesahan HIV bagi kes- kes yang disyaki HIV positif. Pada masa ini protokol pengesahan NARL termasuk menjalankan beberapa ujian-ujian saringan untuk menguji semula sera-sera yang telah dirujuk sebagai HIV positif. Jika ujian-ujian ulangan didapati positif, ujian teknik Western blot dijalankan. NARL iuga menggunakan ujian Immunofluorescent (IF) sebagai essei pengesahan tambahan tetapi penggunaan essei ini telah diberhentikan. NARL juga boleh mengesahkan sama ada pesakit menghidapi HIV-1 atau HIV-2.

Asai-asai yang telah disebut di atas adalah untuk mengesan antibodi kepada HIV di dalam pesakit. Bagaimanapun pada peringkat awal jangkilan HIV, antibodi tidak boleh dikesan. Bagi situasi seperti ini, NARL boleh mengesan kehadiran virus dengan mengesan HIV antigen menggunakan teknik ELISA di dalam sera pesakit yang disyaki telah berjangkit virus HIV. NARL juga sedang dalam proses mengasaskan ujian polymerase chain reaction (PCR) untuk mengesan HTV

Dari sini dapat dilihat bahawa NARL menawarkan ujian-ujian yang terperinci bagi diagnosa jangkitan HIV di rlegala irli clan melupakan bahagian penting dalam langkah negara mengawasi penyebaran penyakit berbahaya AIDS.

'Soil-transmitted helminthiases' adalah merupakan salah satu jangkitan cacing yang utama di negara- negara dunia ketiga. Secara umumnya kelaziman penduduk Malaysia mengalami jangkitan ini ialah lebih kurang 40%. Kajian keatas kadar jangkitan ini adalah menekankan kepada kumpulan ethnik. Ianya dijalankan di 43 buah kampung setinggan yang melengkungi kawasan 25 lcm di Kuala Lumpur.

Populasi rangka sampel yang diambil ialah 14 ,367 . Kadar respon yang diterima keseluruhannya ialah 69%. Dengan menggunakan anggaran status ethnik penduduk, kawasan kajian telah dibahagikan kepada 3 kumpulan berdasarkan nisbah kepada kumpulan ethnic Cina.

Dari jumlah 9863 contoh najis yang diambil, 5,746 (58%) didapati mempunyai sekurang- kurangnya 1 jenis cacing. Kadar jangkitan yang tertinggi ialah diperingkat umur 11-15 tahun (74%) dan yang terendah ialah 0-5 tahun (47%). Didapati terdapat kesan variasi di dalam kadar jangkitan apabila populasi penduduk dikawal mengikut status ethnik. Kadar jangkitan oleh setiap jenis cacing telah dibandingkan mengikut kumpulan umur dan status ethnik dan didapati telah menurun kepada lebih kurang 30% disetiap kumpulan manakala kadar anggaran status kumpulan ethnik Cina telah bertambah. Keputusan yang didapati mengikut analisa statistik menggunakan secara chi-square ialah (p<0.01).

Kadar jangkitan 58% yang didapati daripada keseluruhan survei ini adalah tinggi daripada yang diar~ggarkan di Malaysia. Merr~andangkan populasi kajian, ini memang telah dijangkakan. Di samping pembangunan, kadar 'soil-transmitted helminthiases' penduduk di kawasan setinggan di Malaysia tidaklah menunjukkan banyak perubahan semenjak 10 tahun yang lalu.

Pendidikan kesihatan yang intensif, pc~sk i ta ran yang bersih dan program rawatan cacing mungkin dapat membawa perubahan masa depan kepada kajian 'soil-transmitted helminthiases'. Kumpulan ethnik hendaklah diambilkira semasa perancangan program kawalan dibuat. -

Toksisiti tiga jenis pyrethroid, d-phenothrin, decamethrin dan permethrin terhadap tungau . LeptotrombidizrmJletcheri (Womersky & Heaslip) telah diuji dimakmal. Kajian mengenai daya ketahanan tungau yang berjangkit dan yang tidak berjangkit dengan kuman skrub tifus, telah dilakukan. Teknik bioassei yang digunakan ialah teknik pipet Pasteur. Keputusan yang diperolehi telah dianalisa dengan ujian-t, log-probit dan analysis varian. Ketiga-tiga racun serangga tersebut menunjukkan kesan yang berbeza terhadap tungau. Racun serangga yang paling berkebar~ ialah d-phenothrin, diikuti dengan decamethrin dan permethrin. Keputusan analysis menunjukkan tiada perbezaan yang bermakna terhadap daya ketahanan diantara tungau yang berjangkit dan yang tidak berjangkit. Analisa oleh log-probit menunjukkan bahawa tungau paling sensitif kepada kcpckatan d-phenothlin yang bertambah dan terkurang sensitif kepada permethrin. Keputusan Kajian menunjukkan bahawa ketiga- tiga racun serangga yang diuji, berpotensi sebagai bahan untuk pengawalan vektor L.Jletcheri. Hasil kajian ini menunjukkan bahawa pada masa hadapan, biosassei bolehlah dilakukan dengan tungau tidak bcrjangkit sahaja, tindakan ini alran mengurangkan jangkitan tifus skrub di samping pekerja yang menjalankan bioassei itu. Tungau yang dikumpul di medan tidak perlu diasingkan kepada yang berjangkit dan yang tidak, sebelum menjalankan bioassei.

Percubaarl dilapangari ela ah dijalankan di dua kawasan Majlis Perbandaran Petaling Jaya untuk mengesan perbandingan terhadap keberkesanan penggunaan racun serangga yang sekarang ini digunakan iaitu malathion 96% TG dan YdCun serangga yang baru iaitu Resigen keatas nyamuk Aedes Aegjpti - pembawa denggi. Mesin Leco HT) 1JT.V tclah d ip~nakan di dalam pcrc~lhazn ini. Bagi malathion, kadar aliran yang digunakan ialah 90 ml/minit pada kelajuan kenderaan 8 kph dan bagi Resigen, kadar aliran ialah 200 ml,/lninit pada kelajuan kellderaan yallg sama. Ae. neglpti dewasa ditempatkan di dalatn sangkar yang berukuran 1' x 1' x 1' yang diperbuat daripada kain jaring dan jentik-jentik ditempatkan di dalam rnangkuk plastik yang berukuran 8 cm garispusat dan 5 cm tinggi, dan mengandungi 200 ml air. Tiga buah sangkar diletakkan di dalam setiap lima buah rumah; satu ditempat letak kereta, satu dibilek rehat dan satu dibahagian dapur. Jarak purata sangkar-sangkar itu dari tepi jalan ialah 9 m, 13 m dan 18 m. Operasi ULV ini bcrmula antara pukul 6.00 p.m. dan 6.30 p.m. Mangkuk-mangkuk dm sangkar-sangkar itu diambil setelah tempoh setengah jam didedahkan kepada racun serangga d a n diperhatikan kemortalannya setelall 24 jam. 'Ovitraps' diletakkan di dalam rumah-rumah itu se te lah sangkar-sangkar- diarnbil d a n pembiakannya diperiksa setiap minggu. Malathion yang leb ih berkesan member ikan kadar kemortalan yang lebih tinggi jika dibandingkan dengan Resigen. Kadar kemortalan Ae. aegypti diluar rumah adalah lebih tinggi daripada di bilik rehat dan bahagian dapur.

untuk penggunaan secara besar-besaran B. th ~r njgiensis H - 14 ialah penghasilan agen ini dalam kuantiti yang besar seboleh-bolrhnya dengan menggunakan hahan-bahan buangan tempatan dengan perbelanjaan yang kecil, maka kajian yang dijalankan ini dilaksanakan untuk menyiasat pola kinetik per tumbuhan B. thtlrigiensis H-14 isolat Malaysia (dinamakan sebagai IMR-BT-8) dalam herbagai media buangail dan untuk mengenalpasti bahan yang ses~lai untuk pengeluaran. Kajian fermentasi telah dijalankan dalam balang goncang 1 liter mengandungi media huangan yang djbancuh dengan perlgguricarlg bcrputal pada 30 C. Pada selang masa berkala, 2ml sampel dikeluarkan untuk penentuan ciri-ciri pertumbuhan. Hasil fermentasi terakhir telah dituai, ditulinkan dan keupayaannya telah ditentukan melalui bioassai dengan nyamuk. Lima jenis bahan buangan iaitu bahan buangan ltelapa yang diparut halus, buangan l a c a n e soya sisa minyak kelapa sawit. buangan ikan (sisa ikan bilis? dan bran beras telah dikaji. Dalam media nutrien paiwai dan buaiigan kacailg soya, IMR-BT-8 mengambil masa 37 j a ~ ~ l L I I ~ ~ L I ~ matang. Dalam buangan kclapa parut, buangan ikan dan bran beras, bakteria mengambil 28 jam, 26 jam dan 126 jam masing- masing untuk inatang. Endotoksin dituai daripada media nutrien piawai pada 55 jam dan 50 jam daripada buangan kacang soya, buangan kelapa parut dan ikan. Endotoksin hanya dapat dituai 150 jam selepas inokulasi daripada media bran beras. Bagaimanapun tiada pertumbuhan bakteria dikesan dalam sisa minyak kelapa sawit. Perkembangan sporulasi sebagaiinana di- pcrhatikan di hawah mikrosliop menunjukkan bahawa endotoksin telah disintisiskan antara 8 - 24 jam dalam piawai penapai kacang soya, buangan kelapa dan ikan. Toksin tersebut hanya disintisiskan 26 jam selepas inokulasi dalam bran beras. Perubahan pH media penapai juga dicatatkan. Bergantung kepada media yang digunakan, endotoksin tersebut dituai pada suatu julat pH 5 . 5 - 8.5. Dalam ha1 pengeluaran endotoksin dan biojisim, Buangan ikan merupakan media yang sesuai untuk pengeluaran secara besar besai-an IMR-BT 8.

Lapan puluh enam orang pesakit yang disyaki menghidap kanser paru-paru primer dan dirujukkan ke Pusat TB Negara untuk pemeriksaan bronkoskopi gentian-optik telah dipilih dalam kajian ini. Kajian bertujuan untuk menentukan

nilai relatif pelbagai jenis spesimen sitologi dari saluran pernafasan pesakit dalam menentukan diagnosis dan mcngenal pasti faktul-faktui yang mempengaruhi kelupasan sel-sel malignan. Kahak pra-bronkoskopi dan berusan bronkial paling kerap menunjukkan sel-sel diagnostik. Manakala tumor hilar daripada bronkus besar, skuamos dan sel kecil jenisanaplastik pula paling kerap menggelupaskan sel-sel. Kajian menonjolkan keberkesanan kaedah-kaedah sitologi dalam diagnosis kasinoma bronkogenik. Penilaian sitologi mungkin satu-satunya kaedah mengesahkan diagnosis apabila keputusan analisis tidak dapat dirumuskan daripada biopsi. Dalam penyiasatan seseorang pesakit yang disyaki menghidap karsinoma paru-paru, kahak 'deep cough-up' yang baik harus diperiksa terlebih dahulu untuk menentukan diagnosis sebelum pesakit terpaksa melalui prosedur-prosedur seperti bronkoskopi.

Penghasilan antibodi monoklonal untuk menentang bisa ular Naja naja kaouthia.

Rawatan kepada keracunan bisa ular bergantung rapat kepada ketepatan identifikasi jenis ular. Pemajuan kepada huraian yang lebih khusus dan sensitif boleh diperolehi dengan meng- gunakan antibodi monoklonal yang khusus. Dalam kajian ini dua antibodi monoklonal (CV2D8 dan C V l E 9 ) telah dihasilkan terhadap bisa monocellate. Kedua antibodi monoklonal (IgG sub kclas) bcrtindak dcngan berat molekul tinggi 73 Kd berkuasa tarikan dengan antigen bisa ular senduk. Kajian reaksi-silang dengan antigen bisa ular yang lain menunjukkan bahawa monoklonal ini adalah khusus kepada monocellate bisa senduk (Najd naja kaouthia) dan adalah bermanfaat sebagai bahanuji untuk pen- diagnosaan.

Dianggarkan ada di antara 10 hingga 12 juta kes kusla di dunia l~ar i ini. Kawalan kusta melalui pengesanan kes dan rawatan telah tidak berjaya dilakukan. Justeru, peningkatan program kawalan yang sedia wujud dan penemuan strategi alternatif adalah perlu. Vaksinasi ialah salah satu strategi alternatif yang boleh terokai dalam kawalan kusta.

I'emeriksaan proliferasi (ukuran imunit i selmediated 21.1 rlztro) llmfosit dari mereka yang divaksin, sebagai respons kepada antigen MLSA dan PPD melihatkan garnbaran yang sama. Peningkatan jelas pada respons proliferatif kepada MLSA dilihat pada kumpulan yang mendapat dua dos vaksin yang vaksin tertinggi tiga bulan selepas divaksinasi dan sepanjang jangkamasa setahun kalian.

Tujuan kajian ini ialah untuk nlenyiasat respons imun, humoral dan juga sel mediated, kepada vaksinasi dengan vaksin M. leprae armadillo yang dibunuh dengan suhu tinggi, ke atas sukarelawan sihat yang tinggal di negara tidak endemik. Disamping itu, kajian ini juga membuat penyelidik respons kepada M , lepvne di kalangan pesakit kusta, sebagai perbandingan.

Kesan vaksinasi dengan vaksin M. lepme armadillo yang terbunuh, kc atas sukal r lawa~l yang bervaksinasi BCG diantara 23 ke 29 tahun, dikaji. Sukarelawan ini dimasukkan ke dalam 4 kumpulan secara rambang dan diberikan dos vaksin menaik di antara kumpulan iaitu 1.5 x lo7, 5 x lo7, 1.5 x 108 dan 5 x 108 bacilli. Satu kumpulan kontrol yang tidak disuntik vaksin juga dikaji.

Respons imun sel-mediated kepada vaksin ini diukur in uivo melalui "skin test". Respons "skin test" kepada antigen M.lepme larut (MLSA) dan PPD (Ujian Mantoux; PPL) ialah protin dari filtrat kultur M. tziberculosis~ diukur sebelum dan 3 bulan selepas vaksinasi mtlnunjukkan yang kumpulan-kumplilan yang menerima t igz dos vaksin tertinggi menampakkan peningkatan respons yang jelas kepada MLSA. Tidak banyak perbezaan, dilihat pada respons kepada PPD, yang tinggi sebelum dan selepas vakisinasi. Ini adalah sesuatu yang dijangka kerana sukarelawan ini telah divaksinasi dengan BCG sebelumnya.

Kajian respons sel-mediated kepada vaksinasi i n i s e t e r ~ ~ s n y a c l i p ~ r ~ a n j a n g k a n dengan penghasilan klon T sel dari 5 orang sukarelawan yang telah menerima dua dos vaksin tertinggi. Teknik mengklon sel T menlbolehkan satu-satu sel '1' yang mempunyai ciri-ciri tertentu dibiakkan dengan secukupnya untuk kajian seterusnya. Sejumlah 42 klon T sel dibiakkan dan 11 dari klon ini mempunyai respons kepada M. l epra~ dan tidak kepada PPD atau BCG. Lima daripada klon yang spesifik kepada M. leprne mempunyai respons kepada protin M. leprae 18KD. Semua klon ini merupakan fenotip CD4+ dan ada beberapa yang mengeluarkan Interleukin-2 dan Interferon-gamma dan ada yang sitoksik kepada makrofaj jika M. lepme ada bersama.

Respons humoral kepada vaksinasi di kalangan sukarelawan ini juga dikaji. Walaupun dua kumpulan yang menerima dos vaksin terendah tidak menghasilkan tahap antibodi yang tinggi sehingga satu tahun selepas divaksinkasi, dua kumpulan yang menerima dos vaksin tertinggi menghasilkan peningkatan titer antibodi enam bulan selepas divaksinasi.

Respons irnun kepada M. lepms di kalangan pesakit kusta juga dikaji. Pemeriksaan respons limfoproliferatif di kalangan pesakit lepromatus

yang telah lama dirawat (>20 tahun) (TLL) dan yang tidak dirawat CULL) menunjukkan yang beberapa pesakit TLL mempunyai respons keparla M. lepme. 13 klon T sel yang dibiakkan dari 2 daripada pesakit ini menggunakan teknik piawai dan dibandingkan dengan 5 klon T sel yang dibiakkan dari pesakit kusta tuberkuloid (TT). 7 di antara klon ini dikaji secara lebib terperinci, dan adalah didapati yang sementara kesemua klon TT mempunyai respons hanya kepada M. lepme, 3 diantara klon TLL mcmpunyai respons yang spesifik kepada M. leprae dan satu daripada klon ini menunjukkan "broad cross-reactivity"

kepada mikobakteria lain. Kesemua klon ini adalah dari fenotip CD4+.

Kesi~npulan dari kajian ini ialah vaksin M. leprae armadillo yang dibunuh oleh suhu tinggi sudah bersedia untuk diuji di negara endemik di mana efikasinya akan d iukur berdasarkan ke- mampuannya memper t ahankan diri dari dihinggapi kusta. Di samping itu, kajian ini merupakan fasa pertama dalam penggunaan biologi molekular bagi menyingkap imunologi selular penyakit kusta.

Anugerah Saintis Muda dan Teknologis Perubatan IMR

Majlis penyampaian anugerah kepada pemenang- pemenang Anugerah Saintis Muda dan Teknologis Perubatan IMR telah dirasmikan oleh Ketua Pengarah Kesihatan Malaysia, Tan Sri Dr Abdullah bin Abdul Rahman pada 9 Ogos 1991.

Inilah kali pertama anugerah seumpama ini diberikan oleh Institut. Anugerah ini adalah untuk menghargai peranan penting kakitangan makmal dan untuk menggalakkan saintis muda dan teknologis makmal mempertingkatkan usaha dalam penyelidikan yang dapat menyurnbang ke arah perkhidmatan diagnostik yang lebih baik bagi penjagaan kesihatan dan langkah- langkah pencegahan yang lebih berkesan dalam program-program kawalan. Dalam pemilihan

pemenang, Jawatankuasa Pemilihan IMR telah mengkaji semua kertas penyelidikan saintifik yang telah diterbitkan dan menilai kertas ini berdasarkan kecemerlangan saintifik, manfaat hasil penyelidikan dan sumbangan kepada pengetahuan atau maklumat baru.

Pemenang pertama Anugerah Saintis Muda IMR tahun 1991 ialah En Lee Han Lim, kedua Dr Chiang Geak Lian d a n kctiga nr Hsrvinder Kaur Gill. Anugerah Teknologis Perubatan IMR pula dimenangi oleh Cik Eng Kah Lee, tempat kedua Cik Tan Geok Wah dan ketiga En Abdul Kazak bin Mustaffa.

Kongres Asia Keenam Pemakanan 1991

Kongres Pemakanan Asia diadakan setiap empat tahun sekali dan Kongres yang pertama telah diadakan di New Delhi pada tahun 1971 dan kongres-kongres seterusnya di Manila, Jakarta, Bangkok dan Osaka.

Pada Kongres 1987 di Osaka, Jepun, Persatuan Pemakanan Malaysia (NSM) telah dengan beraninya menyahut cabaran untuk menjadi tuan rumah Kongres Pemakanan Asia yang Keenam. Kongres ini telah diadakan di Pusat Dagangan Dunia Putra, Kuala Lumpur pada 16-19 September, 1991. Memandangkan Persatuan ini masih baru iaitu ditubuhkan pada tahun 1985 dengan hanya mempunyai 300 orang ahli dan akaun bank yang sedikit, tugas yang hendak dipikul ini adalah amat mencabar. Walau bagaimanapun,

berkat dedikasi ahli-ahli dan bantuan kewangan dan sokongan kumpulan-kumpulan penaja, Komiti Penganjur telah berjaya mengatasi kebanyakan rintangan yang telah dihadapi. NSM telah berjaya mengadakan program saintifik yang seimbang dan menarik bagi mencerminkan tema kongres iaitu "Nutritional Challenges and Frontiers Towards

Year 2000".

Kongres ini telah dihadiri oleh 700 orang peserta dari 45 buah negara dan bilangan negara yang mengambil bahagian mungkin adalah yang terbanyak berbanding dengan kongres-kongres dari sebelumnya P e n g a n j i ~ r - p e n g a n j u r juga

menaja penuh atau separuh bagi 50 orang peserta asing. Ini dipercayai belum pernah terjadi di kongres-kongres Pemakanan Asia yang terdahulu.

Program saintifik empat hari ini mengandungi 7 ucapan jemputan yang lengkap, 120 syarahan , simposium, 130 pclnbcntia~lgan kvnlul~ikasi bebas dan 158 poster skop lengkap bagi program saintifik. Oleh itu, empat sessi simposium/ komunikasi bebas terpaksa diadakan secara serentak dalam sehari. Ini menyebabkan ahli- ahli komiti saintifik dan kumpulan odio-visual menjadi amat sibuk.

Pada keseluruhannya, Kongres ini amat berjaya di dalam pengurusan Kewangan dan juga kualiti kandungan saintifiknya. Ini telah dicerminkan

dari maklumbalas positif yang diterima dari peserta-peserta luar negara. Lapuran perjalanan kongres ini boleh didapa~i di sekretaria~ NSM, d/a Bahagian Pemakanan, Institut Penyelidikan Perubatan, Kuala Lumpur.

Kongres Pemakanan Asia yang ke Tujuh telah dijadualkan akan diadakan pada September, 1995 di Beijing, China.

I Rancangan Pengawasan Keresistamran Antibiotik Kebangsaan 1991 -- I

Semenjak 1981, Bahag~an Kaiikuman IPP telah menebirkan maklumat corak kepekaan antibiotik bagi pencilan bakteria Klinikal dalam buletin ini untuk manfrat doktor-doktor. Walau hagaimanapun data yang ditrrbitkan pada mass lalu adalah daripada beberapa buah hospital yang mendapt bantuan perkhidmatan IPP.

Jadual I mmunjukkan kadar ke1-esistanan semua bakreria yang diasingkan kepada beberapa antibiotik yang diuji di 15 buah makmal dalam bulan Ogos dan September sahaja. Jadual I1 menunjukkan keresistanan bakteria tertentu kepada antibiotik sepanjang tahun kerana cin-ciri seperti ini tidak selalu berlaku. Apabila keresistanan seperti ini dikesan makmal-makmal berkrnaan diliehendaki menghantar kultur ini ke IPP untuk disah dan dikenalpasti. Tetapi malangnya makrnal-makmal berkenaangagal berbuat demiliian, oleh rtu laporan mengena~ keresistanan Netssenagonorrhoeae terhadap spectinomycin, penicillin dan MRSA yang 'resistant' kepada vancomycm mungkin tidak sepertl yang digambarkan. Diagnostic Techniques in Malaria Patricia Lim Kim Chooi

Malaria is still one of the most important diseases affecting humankind. In Malaysia, malaria continues to be an important public health problem causing considerable morbidity and sometimes mortality. Plasmoditrnz falcipar~m is the most important parasite.

An important aspect in the management and control of malaria is the ability to diagnose the infection rapidly and accurately. The ability to detect malarial parasites in humans is an important procedure both for individual diagnosis as well as in epidemiological studies. Microscopic examination of blood films for malarial parasites has been the main method of parasite detection. This method is both specific and sensitive as parasitemia of less than 0.0001% can be detected (Bruce-Chwatt, 1984), but a well-trained technician is required to examine each blood smear. It is also time-consuming, expensive and not very suitable for large scale epidemiological studies.

A recent development in the diagnosis of malarial parasites is the Quantitative Buffy Coat Test (QBC Test) for the rapid diagnosis of the malarial parasite by direct observation of centrifuged samples of blood in QBC tubes (Parzy et al., 1990). The QBC test could detect levels of parasitaemia as low as 0.00005%. Studies conducted by the IMR however, showed this cesL to be less superior than the blood film method. In addition, this test cannot quantitate parasitaemia easily and is also expensive to perform for large-scale epidemiological studies.

Over the years a number of serological tests has been developed for the detection of either malarial antibodies or plasmodia1 antigens in sera of malaria patients. Serological assays for antibody detection are more useful as epidemiological tools to assess and monitor malaria endemicity, the level of malaria transmission and the impact of control measures rather than for individual diagnosis as the presence of malarial antibodies

in the serum reflects past as well as current infections.

The gel precipitation test is technically simple and inexpensive, but has been shown to be relatively insensitive and requires high concentrations of antigens. The indirect hemagglutination test (IHA) is one of the earliest test developed for the detection of malarial antibodies. This test is simple and inexpensive and a large number of samples can be processed at a time. However, it lacks sensitivity in detecting early infections.

The indirect fluorescent antibody test (IFAT) has in recent years been the most promising test for malarial antibody detection due to its sensitivity and specificity. Its usefulness as an epidemiological tool has been demonstrated in Malaysia (Mak et al., 1987). This test has proved useful as an indicator of malaria experience of a population, level of endemicity and transmission as well as in the monitoring of control measures for malaria. However, the need for a fluorescent microscope renders this test impraclical in [he field, and reading of the slides by human individuals leads to subjective results. With the subsequent development of the enzyme-linked immunosorbent assay (ELISA), test results can be read visually making it useful in the field. It also yields objective results and can be partially automated and standardized for tcsLi11g a large number of samples at one time (Bidwell and Voller, 1981).

An alternative approach to light microscopy for the diagnosis of the presence of plasmodia in human blood is the detection of parasite antigens or parasite deoxyribonucleic acid (DNA). .The introduction of biotechnology in recent years has facilitated the use of monoclonal antibodies and DNA probes in malaria diagnosis.

Recombinant DNA technology has led to the development of P. falciparum and P. uivax DNA probes for the diagnosis of infections in human blood and mosquito vectors. The level of parasitaemia detectable by these probes is approximately 0.0001% which is similar to the sensitivity of blood film examination. Another approach utilizes species-specific probes for detecting ribosomal RNA (rRNA) of malaria parasites as probes detecting this target may show increased sensitivity due to the high copy number of target sequences and their heterogeneity. However, this method has yet to be tested on patient samples, and the question of stability of target rRNA in field samples needs to be addressed. The potential use of DNA probes as an alternative to blood film examination offers several advantages especially for large scale surveys of malarious populations. Currently most of the DNA probes require radio- labelling thus rendering them impractical for either clinical or field use. It is therefore necessary to find methods for the non-radioactive labelling ur DNA prubes. AL presenl, studies are being carried out at the IMR to develop non-radioactive labelled DNA probes of human and simian malaria parasites for use as diagnostic and sero- epidemiological tools.

Another important development is the introduction of the hyb~iduma t c c l ~ ~ ~ i q u e lur ~11c produclion of monoclonal antibodies. This technique has been widely employed to produce monoclonal antibodies against different stages of the malarial parasite for parasite identification, diagnosis and immunological studies. Monoclonal antibodies to the circumsporozoite proteins of malaria parasites have been used tu d e t c r ~ ~ ~ i ~ ~ e he prevalence of naturally infected mosquitoes, their sporozoite load and the species status of the infecting sporozoite.

Monoclonal antibodies produced against asexual blood stages with defined specificities will be uscful for diagnosis of plasmodia1 antigem in

sera of malaria patients. Such monoclonal antibodies have been used in the ELISA and the immunoradiometric assay (IKMA) for antigen detection. Both these assays showed good sensitivity and the IRMA showed good correlation with parasitaemia with the ability to detect as few as 2.4 malaria parasites per lo8 red blood cells. Monoclonal antibodies developed against P. falcipamm and P. cynomolgi at the IMR have also shown good specificity and sensitivity in the sandwich-ELISA for antigen detection (Lim et al., 1992). Further work to develop dot immuno- binding assays using these monoclonals will facilitate the development of rapid field tests for seroepidemiological studies.

Cryptosporidiosis Among Malaysians Karen Lai Peng Foon

Cvyptosporidium is a coccidian protozoon which is commonly found in vertebrates. Cryptosporidium species may be closely related to each other but only Cyptosporidium paruunz is the one usually responsible for infection in man. The infection is zoonotic and animals like reptiles, birds, horse, calf, pig, and cat act as reservoir hosts. It is transmitted via the faecal- oral route or through contaminated drinking water. The organism is not killed by chlorine nor can it be filtered out using the conventional treatment for water supply to homes.

The organism can be a commensal or an opportunistic, acting as benign or hostile life- threatening pathogen, depending on the immune status of the person. High asymptomatic carrier rate was found among patients undergoing gastroduodenoscopy for other diseases. Cqptosporidium is generally common in children of the low socioeconomic group. It causes severe diarrhoea in children with hypogammaglobulinaemia and cellular immunodeficiencies and in AIDS patients. It is also a traveller's disease as adults often come down with diarrhoea from this infection after a trip to a less developed country. hp~demic of this infection has been reported in USA and in England mainly through water contaminated with animal faeces.

Its importance as a pathogen only came to light within the past 8 years, due to increase awareness of this parasitic disease and improvement in diagnostic techniques. Recently Cyptosporidium has been reported to cause cholestatic syndrome characterised by sclerosing cholangitis and papillary stenosis in AIDS patient.

There is little information about this infection in Malaysia. A study among paediatric patients from the gastro-enteritis ward of the Klang general hospital in 1987 showed a 4% prevalence

of Cvyptospovidiz~m. Another study done at University Hospital showed a 2.1% prevalence. A recent study in Kelantan showed 11.5% positives among young children of seven years and below admitted into hospital because of diarrhoea. Among children with diarrhoea in the community in Kelantan, 10.6% of those submitted stools samples were infected with Cqptosporidium. This organism may be an important aetiological agent of diarrhoea among children in this country. There are no figures on the prevalence of the infection among patients with immunodificiency syndrome in this country but positive cases have been diagnosed among them.

In general, no specific chemotherapy nor immunotherapy for Cyptosporidiun~ infection has been identified but promising results have been achieved with certain drugs namely Octreotide which is a somatostatin analogue, Zidovudine, Halofuginone lactate, Spiramicin, Azithromycin and some Sulphonarnides like Sulfadimethoxin and Sulfamethadine. In most cases the symptoms and infection resolve after a few days, but among those predisposed by other diseases, both symptoms and infection often persist for long periods.

This protozoon is not routinely diagnosed in our clinical laboratories as its importance as a cause of diarrhoea has only being recognised recently. In view of the facts that many of the diarrhoeal cases in our hospitals are caused by this organism and that an effective drug may be soon available, laboratory investigation for this organism should be carried out for all diarrhoeal cases among young children and immunodeficient patients. The modified Ziehl-Neelsen srdining technique is most commonly employed to stain the organisms red in faecal smears making them readily observed under a compound microscope. Immunofluorescent technique using a commercial kit is available.

A Novel Immunoassay Technology, the MEIA Joseph B Lopez

Following the erminal work of Yallow and B Berson in 1960 , immunoassays have followed a path of explosive growth. This has resulted in many variants of the original technique for the measurement of anal~tes present in very small concentrations in biological fluids. Early immunoassays used radioisotopes as label (and were therefore called radioimmunoassays, RIA), were tedious and often imprecise. Later refinements saw better precision and shorter assay times but persisted in the use of potentially hazardous radioisotopes.

A significant milestone in the development of immunoassays was the introduction of enzymes to replace radioisotopes, in the early seventies, a brai~lcl~ild of Swedish researchers Engvall and perlmann233. This had the obvious attractions of safety, reagents with longer shelf-lives and end-point signals which did not require the use of expensive instruments. Alkaline phosphatase and horseradish peroxidase are the labels commonly used in enzyme-immunoassay (EIA).

The need to handle large numbers of samples rapidly and reliably, povided the impetui for the automation of immunoassays. EIAS, in particular, appeared amenable to this development. In clinical chemistry the development of the homogenous Enzyme- Multiplied Immunoassay Technique (EMIT, Syva Corp., USA) in the seventies was a signifirant step in this direction, though its use was confined to the measurement of mostly small molecules such as drugs. The late eighties saw the development of elegant, fully-automated ElAs with dedicated instrumentation. One of these was the micro-particle enzyme immunoassay (MEIA)~.

The MEIA technology uses submicron microparticles coated with a capture molecule (antigen, antibody or viral particle) specific for the analyte bei11g mcasu~-cd. The large surface area and the small diffusion distances between analyte and solid phase result in increased asssay kinetics and decreased incubation times enabling assays to be performed rapidly.

After incubation of the microparticles and analyte, the reaction mixture is transfered to an inert glass fibre matrix to which the microparticles bind irreversibly. The immune complex is retained by the glass fibres while the reaction flows rapidly through the large pores in the matrix. The detection of the immune complex on the glass fibre matrix is accomplished with an alkaline phosphatase labeled conjugate that catalyzes the hydrolysis of 4-methylumbelliferone. The rate at which the 4-methylumbelliferone, a fluoresce111 producl, is generaled on h e rnalrix is proportional to the concentration of the analyte in the test sample.

The instrument used for this assay system is the Abbott IMx (Abbott Diagnostics, N Chicago, USA). The system can be used to measure commonly assayed analytes such as peptide and steroid hormones, tumour markers, other proteins and viral particles and antibodies. The instrl~ment is simple tn operate, fiilly alltomated and, with the exception of loading samples, no human intervention is required. The IMx can assay up to 24 patient samples per analyte-run, wlth each run, depending on the analyte, taking about 35-45 minutes. Manual RIAs and EIAs, by comparison, may take anything from a few hours to up to 3, days per batch.

HIV Testing Facilities at the National AIDS Reference Laboratory, IMR Randel Fang

Facilities for the diagnosis of the Human Immunodeficiency virus (HIV) were established in the Division of Virology, lnstitute for Medical Kesearch (IMR) in 1985. The role of this particular unit was gradually expanded and improved upon, and in 1987, the Ministry of Health (MOH) designated the IMR as the National AIDS Reference Laboratory (NARL) with the HIV laboratory forming a major component of this new facility.

The facilities offered by the NARL for HIV diagnosis can be broadly divided into 3 categories: screening, confirmatory and research. The NARL serves as one of the designated screening centres for antibodies to HIV on behalf of the MOH. However, it differs from the other screening centres by not limiting to the use of only a preselected screening assay. In its current protocol, the NARL utilises two different screening assays routinely, a recombinant competitive enzyme linked immunosorbent assay (ELISA) technique, an agglutination assay using synthetic particles. This protocol is subjected to constant review and update. In addition to using these two techniques, the NARL is engaged in ongoing evaluations of new screening protocols/kits, and, has as one of its functions, the evaluation of all new HIV diagnostic kits prior to their introduction in the local market.

In addition to carrying out HIV screening assays, the NARL also currently functions as the only laboratory

in the country that is authorised to confirm the HIV status of suspected HIV positive patients. The NAWs confirmatory protocol currently involves the retesting of all referred HIV suspected positive sera by several screening assays and then testing all repeatedly positive sera by the Western blot (WB) technique. It also uses an Immunofluorescent (IF) test as an additional confirmatory assay, but this procedure has since been discontinued. The NARL is also able to provide differential confirmatory diagnosis as to whether the patient is suffering from an HIV-1 or HIV-2 infection.

The assays described thus far are to detect the presence of antibodies to HIV in a patient. However, there is a period of time, usually immediately after infection, where antibodies to HIV cannot be detected. In such situations, the NARL is also able to determine if the virus is present in the patient with the detection of HIV antigen in the serum of suspected patients by the ELISA technique. The NARL is also in the process of establishing a polymerase chain reaction (PCR) test for the detection of HIV.

As can be seen, therefore the NARL offers a comprehensive range of tests for the diagnosis of HIV infection in the country, and forms a vital part of the nation's battle to control the spread of the deadly disease of AIDS.

Soil-transmitted helminthiases in squatter population around Kuala Lumpur by ethnic distribution.

Soil-transmitted helminthiases is one of the major worm infections in third world countries. The overall prevalence of soil transmitted helminth infections in the general population of Malaysia is approximately 40% This study emphasises on the rates of infection by ethnic group. The study was conducted in 43 squatter villages located within a 25 km radius of the Kuala Lumpur city centre.

The population in the sampling frame was 14,367. The ovcrall response rate was 69%. Using lie estimates of the ethnic status of the communities, the study areas were divided into three groups based on the proportion of Chinese ethnic group.

In a total of 9863 stool samples collected, 5,746 (58%) had at least one type of helminth present. The infection rates by age w~ere highest in the 11-15 years old age groups (74%) and lowest in the 0-5 (47%). However, we found a marked variation in infection rates when the ethnic status of the pop~lations in the villages were controlled for. Infection rates by individual helminth species is compared among different age groups and ethnic status. There was approximately a SOYO decrease in infection rates in each of the groups as the estimated rate of Chinese ethnic status increased in the community. The results were statistically significant by Chi square (p<0.01). The overall infection rate of 58% in this survey is higher than the estimate of 40% for Malaysia. This is not unexpected, considering the s~udy population. In spite of development, the rate of soil transmitted helminthiasis did not show much change in squatter populations of Malaysia over the last 10 years.

Intensive health education, cleaner enviornment and deworming programmes may bring a change in the future trends of soil transmitted helminthiasis. Ethnicity must not be forgotten while planning for control programmes.

Efficacy of three pyrethreids against Leptotrombidium fletchri (acari: trombiculidae) infected and noninfected with scrub typhus.

Toxicities of three pyrethroids, d-phenothrin, decamethrin, and permethrin, were evaluated in the laboratory against Leptotrombidiunz jletcheri ( Womcrslcy & I Icaslip). Thc Susccptibilitics bctwcen populations of the species infected and noninfected with scrub typhus were investigated. Bioassays for the pyrethroids were conducted by the pasteur pippette technique. Results were analysed by t-test, one-way analysis of variance and log-probit regression. The three pesticides exhibited different toxicities to the chiggers. There were no significant differences between susceptibilities of the infected and noninfected population. Log-probit regression lines indicated that the species was most sensitive to increasing concentrations of d-phenothrin and least sensitive to permethrin. The results show that the three pesticides are potiential candidates for chemical control of L.&tchcri. It may be possible in the future to conduct sirnilar bioassays only with the noninfected population, thus reducing risk of infection to workers conducting the bioassays. Similarly, there may not be a need to separate field-collected chiggers into the two population before performing the bioassays.

A field trial on the comparative effectiveness of malathion and resigen by ULV application on Aedes aegypti.

Field trials were conducted in two residential areas of Petaling Jaya Municipality to determine the comparative effectiveness of the currently used insecticide malathion 96% TG and Resigen, a new insecticide on Aedes neapti - vector of dengue.

The Leco HD ULV machine was used throughout the trials. For the malathion the flow rate was 90 ml/ minute at a vehicle speed of 8kph and for Kesigen

the flow rate was 200 ml/minute at the same vehicle speed. Adult Ae. aegypti were placed in 1' x 1' x 1' cages of fine mesh cloth and larvae were placed in plastic cups measuring 8cm in diameter and 5 cm in height, containing 200ml of water. Three cages were placed in each of the five houses; one in the porch, one in the living room and one in the kitchen. The average distance of the cages from the road edge were 7,13 and 18 meters respectively. ULV operations started between 6.00 pm and 6.30 pm. The test cages and cups were removed after a half hour period of exposure to insecticide and observed for mortality after 24 hol~rs. Ovitraps were placed in the ho~ises after the cages had been removed and checked weekly for breeding. Malathion was more effective giving higher mortality rates when compared with Resigen. The mortality rate of adult Ae. ae~ypti outdoor was higher than in the living room and kitchen. Both insecticides did not show promising larvicidal effects. The ovitraps whcn checked one week after ULV operations showed that 50 % of the traps were positive. From the above results it is evident that for successful control during a dengue epidemic, ground ULV application alone is insufficient. Adult mosquitoes resting in corners and under beds would not be killed by ULV application. Thus ground ULV application combined with thermal fogging, larviciding, source reduction and environmental sanitation should be carried out simiiltaneniisly

Fermentation of a Malaysian bacillus thurirgiensis serotype H- 14 isolate, a mosquito microbial control agent utilizing local wastes.

Bacillus thut-ingiensis serotype H-14 is a highly promising microbial control agent of mosquitoes and blackfly. Its specificity, biodegradibility and larval toxicity have enhanced its widespread use in vector control. Since one of the pre-requisites for mass scale application of B thuringiensisH-14 is the production of large quantities of this agent preferably utilizing local wastes at low cost, this present study was conducted to investigate the growth kinetic pattern of a Malaysian isolate of B thuringiensis H-14 (desigliated as IMR-BT-8) in various waste media and to identify those wastes suitable for production. Fermentation studies were conducted in 1-liter shake-flasks containing the media and agitated by a rotary shaker at 30 C. At periodic intervals, 2-ml samples were withdrawn for the

determination of growth characteristics. The final fermented product was harvested, purified and its potency was determined by bioassays with mosquitoes. Five types of wastes, viz grated coconut waste, soyabean waste, palm oil effluent, fishmeal (remains of dried anchovy) and rice bran were studied. In a standard nutrient broth and soya bean waste, IMR-BT-8 took about 37 hours to mature. In the grated coconut waste, fishmeal and rice bran, the bacteria took 28 11, 26 h a11d 126 11 ~espectively to mature. The endotoxin was harvested from the standard nutrient broth at 55 h and at 50 h from soyabean, grated coconut waste and fishmeal. The endotoxin could only be harvested 150 h after inoculation from rice bran medium. However, no bacterial growth was detected in palm oil effluent. The progress of sporulation as observed under microscope showed that the endotoxin was synthesized between 8-24 h in fermenting standard, soya bean, coconut waste and fishmeal. The toxin was only synthesized 26 h after inoculation in rice bran. Variations in the pH of the fermenting media were also noted. Depending on the media used, the endotoxin was harvested at a pH range of 5.5-8.5. In terms of endotoxin and biomass production, fishrneal appeared to be a suitable medium for the mass scale production of IMR-BT-8.

86 patients suspected of primary lung cancer, referred to the National Tuberculosis Centre for fibreoptic bronchoscopy, were selected for this study to determine the relative value of the various types of respiratory cytology material in establishing a diagnosis and to identify the factors which influence the exfoliation of malignant cells. Prebronchoscopic sputa and bronchial bnishings were fni~nd to yield diagnostic cells most frequently. Hilar tumours arising from large bronchi, squamous and small cell anaplastic types were found to exfoliate cells most frequently. The study highlights the effectiveness of cytologic methods in the diagnosis of bronchogenic carcinoma. Cytologic evaluation may be the only method of confirming the diagnosis whcn biopsy is contraindicated. In the investigation of a patient thought to have lung carcinoma, good "deep cough- up" sputa shuld be first examined to prove a diagnosis before subjecting him or her to procedure like bronchoscopy.

Immune responses to Mycobacterium leprae in healthy humans and leprosy patients Haruinder Kaur Gill

It is estimated that there are between 10 to 12 million cases of leprosy in the world today. The early pl-umist: uT lcprusy curllrul through prompt case detection and treatment has not been realized. There is a need, therefore, to explore both improvements in the existing control programme and in alternative strategies for control. Vaccination is one alternative strategy which could be employed in the control of leprosy.

The aim of this study was to examine the immune responses, both humoral and cell-mediated to vaccination with a heat killed, armadillo-derived AL

lepmevaccine in healthy volunteers living in a non- endemic contry. In addition, the study also examined the responses to M. leprae in leprosy patients for comparative purposes.

The effect of vaccination with a killed, armadillo- dzrived M. Ieprzw vauciilt: was studied in healthy, BCG-vaccinated human volunteers aged between 23 and 29 years of age. The volunteers were randomly assigned to 4 groups and given graded doses of the vaccine ie. 1.5 x lo7, 5 x lo7, 1.5 x l oX , 5 x lo8 bacilli. A non vaccinated control group was also included in the study.

The cell mediated immune response to the vaccine was measured i ~ z uiuo by the skin test. Skin test responses to a soluble M Ieprneantigen (MLSA) and to PPD (Mantoux test; PPD is a protein derived from the culture filtrate of M. tuberculosis) measured before and 3 months after vaccination showed that the groups which recieved the three highest doses of vaccine showed a marked increase in the responses to MLSA. There was little change in their responses toPPD, whichwere high before 2nd aft~rvgccinstion. This was an expected finding as the volunteers had

. been vaccinated with BCG.

The examination of the proliferation (an in vitro measure of cell mediated immunity) of lymphocytes, from the vaccinees, in response to added antigens such as MLSA and PPD revealed a similar picture. A marked increase in the proliferative responses to MLSA was observed in the groups which received the two highest doses of vaccine three months after vaccination and persisted for the study period of a year.

The study of this cell-mediated response to the vaccination was further extended by establishing T cell clones from 5 volunteers who had received the two highest doses of vaccine. The technique of cloning T cells provides a means of amassing sufficient numbers of a particular T cell with a uniquc spccificity for study. A total of 42 T cell clones were raised and 11 of these clones responded to M. leprae and not to PPD or BCG. Five of these M. leprae-specific clones responded to an 18Kd M. leprae protein antigen. All of these clones were of the CD4+ phenotype and a few of them were shown to produce interleukin-2 and gamma-interferon and to be cytotoxic for macophages in the presence of M, leprae.

The humoral responses to vaccination were also studied in these volunteers. While the two groups which recieved the two lower doses of vaccine did not develop any discernible levels of antibody up to 1 year after vaccination, the two groups which received the higher doses of vaccine showed a marked rise in antibody titre six months after vaccination.

Immune responses to M. leprae were also studied in leprosy patients. Examination of the lymphnpmliferative responses of long-treated (>20 years) lepromatous leprosy patients (TLL) and untreated lepromatous leprosy patients CULL)

revealed that a few TLL patients were able to respond to M. leprae. Thirteen T cell clones were raised from 2 of these patients using a standard technique and compared with 5 T cell clones raised from 2 tuberculoid leprosy ('IT) patients. Seven of these clones were studied further and it was found that while all the TT clones responded only to M. leprae, 3 of the TLL clones responded specifically to M leprae and one of these clones exhibited a broad

cross-reactivity to other mycobacteria. All these clones were of the CD4+ phenotype.

This study concluded that the heat killed, armadillo- derived M. leprae vaccine was ready for trial in an endemic country where its efficacy will be monitored by its ability to protect against leprosy. In addition, this study and many others like it represent the first phase of the application of molecular biology to the elucidation of the cellular immunology of leprosy.

IMR Young Scientist and IMR Medical Technologist Awards

General of Health Malaysia, Tan Sri Dr. Abdullah bin Abdul Rahman on 9th August 1991. This is the first time such awards are being given by the Institute. 'l'he creation of these awards 1s to show recognition of the importmt roles played by the laboratory staff and to motivate young scientists and laboratory technologists to greater efforts in research leading to the development of better diagnostic services for health care and more efficient intervention measures for control programmes.

In the selection of the winners, the IMR Selection Committee reviewed all published scientific papers submitted and considered the scientific excellence, utilizability of the research findings as well as contribution to new knowledge of the publications.

The winners of the IMR Young Scientist Award was Mr. Lee Han Lim while the runners-up were Dr. Chiang Geok Lian and Dr. Harvinder Kaur Gill. For the IMR Medical Technologist Award, the winner was Ms Eng Hah Tee and the runners-up were Ms. Tan Geok Wah and Mr. Abdul Razak bin Mustaffa.

The Sixth ASIAN Congress of Nutrition 1991

Asian Nutrition Congresses have been held regularly at four-yearly intervals, beginning with the First Congress in New Delhi in 1971 and subsequently in Manila, Jakarta, Bangkok and Osaka.

At the 1987 Congress in Osaka, Japan, the Nutrition Society of Malaysia (NSM) boldly undertook the challenge to host the Sixth Asian Congress of Nutrition. This event was held at the Putra World Trade and Convention Centre, Kuala Lumpur, 16-19 September, 1991. Being a relatively young society, having only been established in 1985, and with only about 300 members and a meagre bank account at the time, the task ahead for the NSM was indeed formidable. However, thanks to its core of dedicated members, and the financial assistance and support from NSM's co-sponsors, the Organising Committee was able to overcome most of the obstacles encountered and put together a balanced and

interesting scientific programme that reflected on the theme of the Congress- "Nutritional Challenges and Frontiers Towards Year 2000".

Thc Congrcss was attcnded by about 700 participants from 45 countries, and the latter figure was probably a record. The Organisers had also provided full or partial sponsorship for about 50 foreign participants, and this is believed to be without precedence for an Asian Nutrition Congress.

The four-day scientific programme consisted of 7 plenary addresses, 120 symposium lectures! 130 free communications and 158 posters. The rich scope of the scientific programme necessitated the holding of four simultaneous syrnposiumllfree communication sessions daily, and this kept members of the Scientific Committee and the audio-visual team very busy indeed.
